Analyze And Summarize Professor Wang Sheng’s Clinical Experience In The Treatment Of Chronic Obstructive Pulmonary Disease Based On Data Mining Methods

Objective: Based on the analysis of syndrome differentiation in traditional Chinese medicine,combined with data mining technology,summarize Professor Wang Sheng’s clinical experience in the treatment of chronic obstructive pulmonary disease.Method: Through follow-up,the medical records of patients with chronic obstructive pulmonary disease who were in the expert clinic of Professor Wang Sheng from January2019 to December 2020 were collected,and the medical records of 177 cases and 397 outpatient visits that met the criteria were screened out.With the help of the “Digital Platform for Damous Old Chinese Medical Experience” developed by the Information Center of Anhui Provincial Hospital of Traditional Chinese Medicine,the general condition of patients(gender/age/smoking history/quarter visits),symptoms(symptoms/tongue pulse/syndrome type),drugs(use frequency/four qi/wu wei/gui jing)for frequency analysis;for drug-drug,symptom-drug association rule analysis;cluster analysis for core prescriptions and basic core prescriptions corresponding to each syndrome type.The results of data mining are processed and summarized at last,then Professor **’s clinical experience in treating chronic obstructive pulmonary disease is summarized,and three typical clinical cases are cited.Result:1)General situation: 137 males and 40 female patients;The oldest is 95 years old,the youngest is 41 years old,with an average age of 69.29 years old;Smoking history: 25 unknown,50 non-smokers,56 quit smoking,46 people who have not quit smoking;The number of visits in the first quarter of 2019 and 2020 is 72 times in the first quarter,145 times in the second quarter,108 times in the third quarter,and 72 times in the fourth quarter.The average number of visits per patient is 2.24.2)Symptoms: Frequency of symptoms: the frequency of all symptoms ≥50 are:Asthma,Cough,Dry mouth,White sputum,Excessive phlegm,Fatigue,Anorexia,Itchy throat;Tongue and pulse frequency: Tongue is pale red The most common,followed by red tongue and dark tongue;the most common tongue coating is thin and white coating,followed by less coating,thin yellow coating,and white and greasy coating;pulse is dominated by heavy and thin pulse,followed by thin and slippery pulse,and weak pulse,Pulse floating and thin;distribution of syndrome types: phlegm-heat stagnation in the lung,qi and yin deficiency syndrome(37.03%),lung and kidney deficiency syndrome(19.90%),phlegm-heat stagnation in the lung,lung failure syndrome(15.36%),cold Drinking to condense the lung,deficiency of both qi and yang(10.58%),and phlegm-heat stasis syndrome(9.57%).3)Drugs: High-frequency drugs: The top 10 drugs are Ephedra,Dilong,Tinglizi,Zhe Fritillaria,Bitter Almond,Reed Root,Lily,Scutellaria,Citrus aurantium.Adenophora:Calm,Warm,Cool and Hot;The five flavors of the drug are bitter,salty,sweet,pungent,and sour;The frequency of menstrual return is lung,spleen,stomach,heart,liver,large intestine,bladder,kidney,gallbladder,small intestine,pericardium,Sanjiao.4)Drug combination:(1)Commonly used: drug pair Shegan+Ephedra,Bitter Almond+Ephedra,Lily+Southern Adenophora,Shegan+Tinglizi,Stiff silkworm+Cicada slough,Stiff silkworm+Dilong,Ephedra+Tinglizi;(2)Three drugs The correlation is mainly the compatibility of phlegm and cough and asthma medicines,heat-clearing medicines,and tonic medicines;(3)The four-drug correlation mainly embodies the treatment methods of resolving phlegm and relieving cough and asthma,clearing heat,relieving the liver and relieving wind,replenishing qi and nourishing yin,and has both good and evil Compatibility characteristics of attacking and supplementing;(4)The compatibility of commonly used drugs related to five drugs is still mainly based on clearing heat and resolving phlegm,relieving cough and relieving asthma,replenishing qi and nourishing yin,and there are applications of clearing heat and treating COPD:(1)Professor Wang Sheng has rich clinical experience in the treatment of COPD.He believes that "preventing the disease before the disease,preventing the change of the existing disease" is the primary link in the diagnosis and treatment of COPD,and attaches great importance to the treatment of COPD.Patients receive health education;(2)The idea of syndrome differentiation is the core.Teacher Wang pointed out that the core differentiation idea of COPD is the differentiation of deficiency and excess,the differentiation of cold and heat,and the internal organs.Combined use,it has the effect of eliminating the actual evil and replenishing the deficiency;(4)Prescription of the right medicine is an important part of the diagnosis and treatment of chronic obstructive pulmonary disease.2)Modern technology has opened up an effective way to explore the summary of the experience of TCM workers.Data mining technology can analyze TCM diagnosis and treatment information more systematically,and can dig out potential laws in the links of syndrome differentiation and treatment,prescription and medication,and increase its objectivity.It is of great significance to promote the inheritance of TCM technical experience and promote the development and innovation of TCM.
