| Objective:Through clinical study,the clinical efficacy and related mechanism of Tong Du Tiao Shen acupuncture on carotid atherosclerosis in patients with internal carotid artery system TIA were explored,so as to provide a new treatment idea for reducing the incidence of ischemic stroke events.Method:Sixty TIA inpatients were collected in the Department of Encephalopathy of Anhui Acupuncture and Moxibustion Hospital,and randomly divided into the western medicine group and the acupuncture and medicine combination group,with 30 cases in each group.The two groups were treated 6 times a week for 12 consecutive weeks as a course of treatment.Before and after treatment,the included TIA patients were evaluated for TCM syndrome scores of stroke forebode syndrome,and the carotid intima media thickness(IMT),Crouse plaque score,plasma fibrinogen(FIB),plasma homocysteine(Hcy)and blood lipid levels were determined,and the total effective rate was observed.After treatment,the data of the same group were compared before and after treatment.The data of different groups were compared and analyzed respectively,and the data comparison between each group was observed,the results were analyzed and the conclusions were obtained.Result:(1)IMT and Crouse plaque scoresBefore treatment,IMT and Crouse plaque scores were measured in the two groups,which were comparable(P>0.05).After 1 course of treatment,IMT and Crouse plaque scores in 2 groups were lower than those before treatment(P<0.05),suggesting that the two treatment methods have a good effect on the regulation of IMT and plaque morphology of carotid artery.IMT value and Crouse plaque score of patients in the acupuncture and drug combination group decreased more than those in the western medicine group(P<0.05),the difference was statistically significant,indicating that the acupuncture and drug combination group had a better intervention effect on the carotid IMT and plaque morphology.(2)FIBBefore treatment,FIB levels were comparable between the two groups(P>0.05).After 1course of treatment,the FIB value in both groups was significantly decreased(P<0.05),suggesting that the two treatment methods had a good regulating effect on the plasma fibrinogen level of patients.Compared with the western medicine group,the FIB value of the acupuncture and medicine combination group decreased more obviously(P<0.05),the difference was statistically significant,indicating that the acupuncture and medicine combination group had a better intervention effect on the plasma fibrinogen level of patients.(3)HcyBefore treatment,Hcy detection was performed in the two groups,which was comparable(P>0.05).After 1 course of treatment,the Hcy value in both groups was significantly decreased(P<0.05),suggesting that the two treatment methods had a good regulating effect on the plasma homocysteine level of patients.Compared with the western medicine group,the Hcy value of the acupuncture and medicine combination group decreased more obviously(P<0.05),the difference was statistically significant,indicating that the acupuncture and drug combination group had a better intervention effect on the plasma homocysteine level of patients.(4)Blood lipid levelBefore treatment,there was no significant difference in blood lipid levels between the two groups(P>0.05).After 1 course of treatment,the values of TC,TG and LDL-C in 2groups decreased significantly,while the values of HDL-C increased significantly(P<0.05),suggesting that the two treatment methods had a good regulating effect on the level of blood lipid in patients.Compared with the western medicine group,the values of TC,TG and LDL-C in the acupuncture and medicine combination group decreased more significantly,while the values of HDL-C increased more significantly(P<0.05),indicating that the intervention effect of the acupuncture and medicine combination group on the blood lipid level of patients was better.(5)Chinese medicine syndrome scoreBefore treatment,there was no statistical difference in TCM syndrome scores between the two groups(P>0.05).After 1 course of treatment,the scores of TCM symptoms in 2groups decreased significantly(P<0.05),suggesting that both treatments have good intervention effects on patients.In addition,the score of TCM syndrome in the acupuncture and medicine combination group decreased more obviously(P<0.05),the difference was statistically significant,indicating that the acupuncture and medicine combination group had a better improvement effect on the symptoms of patients.(6)The total curative effectAfter 1 course of treatment,Wilcoxon rank sum test was performed for the overall efficacy of TIA patients in the two groups.The total effective rate of the western medicine group was 66.67%,and the total effective rate of the acupuncture and medicine combination group was 90%.The total effective rate of acupuncture and drug combination group was higher(P<0.05),and the difference was significant.Conclusion:1.Tong Du Tiao Shen acupuncture can produce benign intervention on carotid atherosclerosis in patients with internal carotid artery system TIA.2.Tong Du Tiao Shen acupuncture can effectively improve the clinical symptoms of patients with internal carotid artery system TIA,and the mechanism of action may be related to benign intervention of carotid atherosclerosis. |
