Background: In the age of global population aging,the proportion of elderly patients with chronic heart failure climbed year by year,and chronic heart failure has become a world public health problem threatening elderly quality of life.With the development of holistic nursing theory,which calls for providing patients with holistic care that is physically and psychologically spiritual,meeting patients’ spirituality needs is a central area of holistic nursing care.At present,foreign research on the spirituality needs of older chronic heart failure patients is relatively more,while most of our country still only focus on the adjustment of disease treatment,psychological intervention and social of older chronic heart failure patients,less attention is paid to the spirituality needs of older chronic heart failure patients,and there is a lack of an assessment tool suitable for the spirituality needs of older chronic heart failure patients.Objective: To evaluate the applicability of the Chinese full version of the spirituality needs questionnaire(SPNQ)in older patients with chronic heart failure,with a view to providing clinical caregivers with an assessment tool of spirituality needs in older patients with chronic heart failure.And based on this,to explore to analyze the current status of spirituality needs of elderly patients with chronic heart failure and its influencing factors.In the hope of raising the attention of clinical caregivers to the spirituality needs of older patients with chronic heart failure,to provide a basis for spirituality nursing care of older patients with chronic heart failure.Methods: 1.Reliability and validity of the questionnaire: using convenience sampling,336 elderly patients with chronic heart failure who were hospitalized in the Department of Cardiology,the third tri forme hospital,Shiyan City,Hubei Province from February to August 2019 were selected as study subjects according to the inclusion criteria.The total sample of 336 cases was randomly divided into two groups A and B,with 168 cases in each group A and B.an exploratory factor analysis was performed in group A,and a confirmatory factor analysis was performed in group B to test the construct validity of the scale.2.Cross-sectional survey: Based on the sample size of the original survey of 336 cases,convenience sampling was used to select 454 elderly patients with chronic heart failure who were hospitalized in the cardiology department of three tertiary hospitals in Shiyan City,Hubei Province from February to December 2019.As research target.The general information questionnaire,the complete Chinese version of the Spiritual Needs Questionnaire(SPNQ),the Minnesota Heart Failure Scale,the Hospital Anxiety and Depression Scale,the Family Support Self-Rating Scale,and the Daily Living Ability Assessment Scale were used to collect participant data.SPSS22.0 software was used for data analysis.Statistical methods include descriptive analysis,independent sample T test,one-way analysis of variance,Pearson correlation analysis and multiple linear regression.Results: 1.Reliability and validity of the questionnaire: The item-content validity index(I-CVI)value of each item of the Chinese full version of the spiritual needs questionnaire is 0.85~1.00,and the content validity of the total scale(scale-content validity)index,S-CVI)value is 0.875,exploratory factor analysis extracts a total of 6factors,the cumulative variance contribution rate is 67.568%,confirmatory factor analysis each index reaches the ideal value,the model fits well;the total questionnaire Cronbach α coefficient is respectively 0.924,the Cronbach α coefficient of each dimension is 0.671~0.939,and the total questionnaire test-retest reliability is 0.903,indicating that the questionnaire reliability is very good.2.The total score of spiritual needs of elderly patients with chronic heart failure in this study is(39.67±10.38).The order of the six dimensions of spiritual needs is positive giving,being relieved,being reflected in reality,inner lingering,inner peace,faith religion.Univariate analysis showed that the total score of the spiritual needs of elderly patients with chronic heart failure with different disease course,heart function classification and religious beliefs was compared,and the difference was statistically significant(P<0.05).Correlation analysis shows that the spiritual needs of elderly patients with chronic heart failure are directly proportional to the overall quality of life,physical area,emotional area,anxiety,and depression,and the difference is statistically significant(P<0.05),and is inversely proportional to the level of self-care ability.It is statistically significant(P<0.05).The spiritual needs of elderly patients with chronic heart failure were not related to the level of family support(P>0.05).Multivariate analysis: The spiritual needs scores of elderly patients with chronic heart failure can be explained by different religious beliefs,disease course,heart function classification,and emotional domains accounting for 20%.Conclusion:1.The complete version of the Chinese spirituality needs questionnaire consists of 27 items and contains 6 dimensions: positively giving needs,faith religious needs,inner peace and needs,existence reality reflecting needs,existence paraphrase needs,inner connectedness needs,and good reliability,and the complete version of the Chinese spirituality needs questionnaire is suitable for use with older patients with chronic heart failure.2.Elderly patients with chronic heart failure had higher spirituality needs,and health care workers should pay special attention to actively giving needs.Older patients with chronic heart failure who had religious beliefs,longer disease duration,higher cardiac functional class,lower quality of life,lower physical domain,lower emotional domain,higher anxiety and depression,and lower level of self-care ability in life had higher spirituality needs.The presence or absence of religion,disease duration,cardiac functional class,and emotional domain were significant influencers of spirituality needs in older patients with chronic heart failure.Special attention should be paid to the spirituality needs of older patients with chronic heart failure who have religious beliefs,a long disease course,a high degree of heart failure,and a low emotional state,so as to provide patients with targeted spirituality care.
