| Objectives:(1)Assess the quality of current life on elderly patients with chronic heart failure.(2)Explore and analyze whether some interrelated factors are related to the quality of life and to improve the quality of elderly patients with chronic heart failure.(3)Research the health needs of the elderly patients with chronic heart,and provide valuable and real reference material for improving elderly chronic heart failure medical service system.Methods:Cross-section survey was used to select elderly patients with chronic heart failure and met the inclusion exclusion criteria in one hospital from August 2018 to June2019,survey and resurvey of general demographic data,clinical indicators,quality of life,and health needs based on patients informed consent.And the investigative tool included:General demographic questionnaire,Quality of life questionnaire of Minnesota heart failure and health demand questionnaire of elderly groups in Anhui province.Two professional medical-related workers used double-entrying for data input,and selected SPSS19.0 software package to establish a database for statistical analysis.Results:1.The mean quality of life score of 139 elderly patients with chronic heart failure in this study was 50.49±7.17.Differences in quality of life scores between patients of different genders,NT-pro BNP、LEVF、NYHA、left ventricular end-diastolic diameter,and etiology of heart failure were significant in statistical.(P<0.05).2.The health needs are mainly represented by health or chronic disease managementservices,medication guidance,family doctors,wearing or using assistant devices,periodic medical examination services,and basic knowledge of the disease.The proportion is 129 cases in order(92.90%),117 cases(84.17%),106 case(76.26%),94cases(67.63%),83 cases(59.71%),and 77 cases(55.40%)were indicated.3.Univariate analysis showed that gender,age,occupation,BNP,LEVF,NYHA,left ventricular end-diastolic diameter,CCI,and length of hospital stay were related to patients’ QOL(Quality Of Life).4.Multivariate analysis showed that gender,occupation,NT-pro BNP,NYHA,CCI,etiology of heart failure,and length of hospital stay were related to patients’ quality of life.5.The analysis of chi-square results showed that there was a significant difference between men’ and women’ needs for dietary guidance and rehabilitation guidance,which was statistically significant(P values were <0.001 and 0.049,respectively);the demand for medication guidance and chronic disease card building was different at different ages.Significantly,there was statistical significance(P values were<0.001,0.024 respectively);the difference in needs of patients with different grades of cardiac function in wearing assistive devices and rehabilitation was statistically significant(P values were<0.021,0.013).In terms of the need for treatment referral and assistant devices,patients with quality of life have significant differences in their needs(P=0.026、0.021),which is statistically significant.Conclusions:1.Elderly patients with chronic heart failure with different demographic data and clinical indicators have different quality of life.2.Gender,occupation,amino-terminal B-type natriuretic peptide precursor level,NYHA grade classification,etiology of heart failure,CCI index and length of hospital stay are the influencing factors for the quality of life of elderly patients with chronic heartfailure.3.Elderly patients patients with chronic heart failure mainly need chronic disease management service,medication guidance,family contracted doctor,wearing and using auxiliary equipment,regular physical examinations,and basic knowledge education of the disease.4.There are differences in the health needs of elderly chronic heart failure patients with different genders,age groups,cardiac function classifications,and quality of life. |
