Objective: To study the clinical significance of CX3CL1 chemokine in primary Sjogren’s syndrome with interstitial lung disease(p SS-ILD)and its correlation with KL-6 in ILD,and to analyze the correlation between CX3CL1 chemokine and clinically relevant indexes,lung function,CT semi-quantitative score.Methods : 1.A total of 42 patients with PSS-ILD,61 patients with PSS and46 healthy subjects from the Health Examination Center were included in the study from December 2019 to December 2020 in the Department of Rheumatology and Immunology,Affiliated Hospital of guilin Medical College.Primary Sjogren’s syndrome was included using classification criteria based on the2016 American College of Rheumatology(ACR)/European League Against Rheumatology(EULAR)PSS classification criteria.ILD is determined by high resolution CT(HRCT).2.The basic data,clinical manifestations,biochemical indexes,lung function,lung CT and serum samples were collected from the enrolled patients.The levels of CX3CL1 and KL-6 in serum of PSS-ILD patients,PSS patients and healthy normal people were detected by ELISA method,and the correlation between CX3CL1 and the clinical-related indexes of PSS-ILD and PSS patients were analyzed and compared.2.The basic data,clinical manifestations,biochemical indexes,lung function,lung CT and serum samples were collected from the enrolled patients.The levels of CX3CL1 and KL-6 in serum of p SS-ILD patients,p SS patients and healthy normal people were detected by ELISA method,and the correlation between CX3CL1 and the clinical-related indexes of p SS-ILD and p SS patients were analyzed and compared.Results: 1.The ratio of male to female in both PSS group and p SS-ILD group was in the range of 1:9-10,and there was no statistical difference in gender and course of disease between p SS group and p SS-ILD group(P > 0.05).The age of p SS-ILD group was significantly higher than that of p SS group,and the mean age between the two groups was more than 50 years old.2.There were statistically significant differences in CRP,globulin,pulmonary hypertension,Essdai score,uric acid,chest shortness of breath and dry cough between PSS group and p SS-ILD group(P < 0.05).There were no significant differences in WBC,RBC,HGB,PLT,complement C4,ESR,RF,creatinine,urinary protein and corneal fluorescence staining between the two groups(P > 0.05).3.Antibodies: There were no significant differences in ANA antibody,anti-SSA antibody,anti-SSB antibody,anti-RO52 antibody,anti-AMA-M2 antibody and anti-RNP antibody between the two groups(P > 0.05).4.Lung function FVC% Pred,FEV1%Pred,TLC% Pred,TLC% Pred,DLCO% Pred had statistical differences between p SS group and ILD group(P < 0.05),FEV1/FVC and RV% had differences between the two groups,but there was no statistical difference(P > 0.05).5.There was a positive correlation between CX3CL1 and KL-6,with statistical significance(P < 0.05).6.CX3CL1 was correlated with DLCO% PRED,HRCT,pulmonary arterial hypertension,Essdai score,chest sulking and shortness of breath,dry cough,and the difference was statistically significant(P < 0.05).7.Uric acid(OR 0.992,95%CI 0.987-0.992,P=0.004),KL-6(OR 0.95,95%CI 0.907-0.994,P=0.026)and Essdai score(OR 0.765,95%CI0.673-0.870,P=0.000)were independent risk factors for the occurrence of p SS-ILD.Conclusions: 1.Age,course of disease,CRP,globulin,uric acid,pulmonary hypertension and pulmonary function can be used to evaluate the prognosis of PSS-ILD and PSS,and Essdai can evaluate the disease activity.2.Serum CX3CL1 can be used for the diagnosis of PSS-ILD and can evaluate the prognosis of the disease to a certain extent.3.Uric acid,KL-6 and Essdai scores are independent risk factors for ILD in PSS patients.
