objective Objective to analyze the relationship between neutrophil to lymphocyte ratio(NLR)and pulmonary interstitial lesions(ILD)in patients with primary Sogren’ssyndromej(PSS),and to explore the clinical significance of NLR in ILD of PSS patients,so as to provide reference for early diagnosis of ILD of PSS patients.method This study retrospectively analyzed the clinical data of 80 patients with primary PSS admitted to the Department of Nephrology and rheumatology,the First Affiliated Hospital of Dali University from January 2018 to may 2020.According to the diagnostic criteria of ILD,the patients were divided into PSS ILD group(n = 29)and PSS non ILD group(n = 51).The basic data,clinical symptoms,laboratory indexes and NLR levels were analyzed and compared.Multivariate logistic regression was used to analyze the influencing factors of ILD in patients with PSS;receiver operating characteristic curve(ROC)was used to analyze the predictive value of NLR for ILD in patients with PSS.Spss25.0 software was used for statistical analysis,and P < 0.05 was taken as the difference with statistical significance.Result 1.In this retrospective study,the incidence of ILD in PSS patients was 36.25%.There was no significant difference in gender,age and course of disease between PSS ILD group and PSS non ILD group(P > 0.05).2.In terms of clinical symptoms,there were significant differences in cough,expectoration and chest tightness between PSS ILD group and PSS non ILD group(P < 0.05).There were no significant differences in dry mouth,dry eyes,rampant dental caries,joint swelling and pain,fatigue,muscle soreness,skin damage and chest pain between PSS ILD group and PSS non ILD group(P > 0.05).3.NLR,ESR,ALB,crea and LDL-C were significantly different between the two groups(P < 0.05).NLR,ESR,crea and LDL-C in PSS ILD group were higher than those in PSS non ILD group,and ALB in PSS non ILD group was higher than that in PSS ILD group.4.Multivariate logistic regression analysis showed that NLR(or: 2.711,95% CI:1.125-6.529,P = 0.026)might be an independent risk factor for ILD in PSS patients.5.ROC curve analysis showed that the area under the ROC curve was 0.704,and its value ranged from 0.7 to 0.9,suggesting that NLR had medium predictive value for ILD in PSS patients(P < 0.05).When NLR was 1.10,it was the best cut-off point for PSS ILD patients,with sensitivity of 96.6% and specificity of 35.3%.Conclusion 1.NLR may be an independent risk factor for ILD in PSS patients.2.NLR has a certain predictive value for ILD in PSS patients.3.NLR of 1.10 is the best diagnostic threshold for ILD in PSS patients. |
