ObjectiveGuided by the theory of patient classification system,drawing on the results of previous nursing workload evaluation tools,translated CUDYR-DIAL scale into Chinese version,and investigate the nursing workload in hemodialysis unit.The purpose of this study focus on the nursing workload of hemodialysis patients during dialysis,and to provide clinical nursing staff with new nursing workload evaluation tool to standardize and reasonably evaluate hemodialysis patients and provide guidance for clinical grading nursing services.MethodsThe Chinese version of CUDYR-DIAL(Categorización de usuario según dependencia y riesgo en unidades de hemodiálisis,CUDYR-DIAL)was formed by translating the original scale.Then the Chinese version of CUDYR-DIAL,simple clinical score scale and Barthel index scores was collected at the same time to exam the reliability and validity of the scale.SPSS 22.0 statistical software was used for data analysis.Finally 1 item was deleted,13 items were retained to further research.The influencing factors affecting the degree of dependence and risk in hemodialysis patients were analyzed to classify group.The 231 patients in 4 hemodialysis centers in Zhenjiang City using stratified sampling method to set the cut-off value and describe patient characteristics,percentage norm and a standard norm was established.ResultsItem analysis indicated that the differences of each item in the high and low groups were statistically significant(p < 0.05)except for scale item 11.The Chinese version of CUDYR-DIAL scale included a total of 2 dimensions and 13 items.The Cronbach’s α coefficient of the total scale in this study was 0.765.Three common factors were extracted by exploratory factor analysis with total 61.629% variance contribution.The total score of dependence on the Chinese version of the CUDYR-DIAL scale was negatively correlated with the total score of the Barthel Index score(r = 0.705,p < 0.001),and the risk degree score was positively correlated with the simple clinical score scale score(r = 0.343,p < 0.001),and the difference was statistically significant.The degree of dependence and risk of patients assessd by3 trained nurses,the result indicating that each scores tended to be consistent,owing to kendell coefficients were 0.956 and 0.988 respectively.In addition,due to the abridgement of scale items,this study tried to establish the regional norm,preliminarily identified the influencing factors through quantile regression,and further established the regional norm of Zhenjiang City,including coarse score norm,standardrized score norm,and grouping norm.As describe the characteristics of hemodialysis patients,most patients were at high risk and mild dependence(24.55%),followed by high risk and severe dependence(15.59%).ConclusionA Chinese version of the CUDYR-DIAL scale,the patient classification tool with good reliability and validity was established according to the factors of patient dependence and risk.It zestablished to provide a prerequisite for the construction of patient classification system.Influencing factors were identified by quantile regression,norm constructed to providing delimitation criteria for the clinical use of the Chinese version of CUDYR-DIAL and further classifying patients. |
