Metabolic-associated fatty liver disease(MAFLD)is a liver metabolic stress disease characterized by excessive liver fat deposition and insulin resistance(IR)as the main pathogenesis.It was named non-alcoholic fatty liver disease(NAFLD)until 2020.In recent years,the incidence rate keeps rising,and the incidence of adverse cardiovascular events and liver cancer related to it has also increased,causing widespread concern in the society.Due to the high degree of heterogeneity of MAFLD,the current effective treatment is based on lifestyle intervention with diet as the cornerstone.Low-carbohydrate diet(LCD)has been widely used in patients with obesity and type 2diabetes.Numerous studies have confirmed that LCD has significant effects on weight loss,lipid reduction,and insulin resistance.Objective:This study mainly explored the effect of LCD on liver steatosis and liver inflammation in patients with MAFLD;the effect of LCD on cardiovascular disease risk factors in patients with MAFLD;and the effect of LCD on renal function in patients with MAFLD.Methods:Collect retrospectively the case data of 914 patients participating in the "Thousand Talents Plan" project,and sort out the anthropometrics(W,BP,WHR,BFR,VFR),liver function(ALT,AST,GGT)of each patient at baseline and 6 weeks,renal function(BUN,SCr),blood lipids(TG,TC,LDL-C,HDL-C),and liver B-ultrasound.According to the inclusion criteria,exclusion criteria and the diagnosis criteria of MAFLD,477 patients with MAFLD were included in the study.According to whether the patients underwent LCD intervention,they were divided into an intervention group(n=244)and a control group(n=233)(the intervention group was the LCD group,and the control group was the conventional diet group).Use SPSS 25.0 for statistical analysis:(1)compare the differences in baseline data between the two groups;(2)compare the baseline and 6-week data of the two groups of patients,and analyze the changes in clinical indicators;(3)compare the differences between the two groups of patients Differences,analyze the differences in the effects of LCD and regular diet on patients with MAFLD.Results:1.It was found that the intervention group has a more significant effect on improving liver steatosis(50.4% vs 24%)by comparing liver steatosis and inflammation-related indicators between the two groups;the intervention group could significantly reduce ALT,AST,GGT,and NFS(P<0.001).2.It was found that the intervention group can significantly reduce W,BMI,WHR,BP,VFR,BFR,TC,TG,LDL-C,SUA,HOMA-IR,CMI(P<0.001)by comparing indicators related to cardiovascular risk factors between the two groups.3.Changes in SCr、BUN had no difference between groupsConclusions:1.LCD could improve liver inflammation and liver steatosis in patients with MAFLD.2.LCD could reduce weight loss,blood lipid reduction,and improve cardiovascular risk factors related indicators significantly.3.LCD may have no effect on SCr and BUN.
