The Concept Construction Of Catastrophic Belief And The Establishment And Application Of Scale

Object:To construct the concept of catastrophic belief the scale of catastrophizing belief was initially developed.To test the reliability and validity of the catastrophic belief scale in anxiety,depression and obsessive-compulsive disorder.Based on the catastrophic belief scale,the specific catastrophic belief assessment technique will be developed,which will lay the foundation for the establishment of the targeted therapy strategy for the catastrophic belief.Methods:The concept of catastrophic belief was determined through literature analysis and semi-structured interview,and the items of the scale were obtained through this process.The scale of catastrophic belief was initially developed by means of item analysis,exploratory factor analysis and confirmatory factor analysis.The initial catastrophic belief scale was tested on 200 subjects to test the internal consistency reliability,split half reliability and test-retest reliability of the scale in patients with mental disorders such as depression,anxiety,and obsessive-compulsive disorder.Confirmatory factor analysis was carried out to investigate the structural validity of the scale and the validity of the scale.After the measurement of the catastropific belief scale,Catastrophizing interview were conducted with 43 subjects.The subjects self-reported the topic of catastropification,and the number of steps of catastropification was generated and the possibility and severity were evaluated through the interviews.A correlation analysis and a comparison of the differences were made between the results of the catastrophic belief scale and the quantitative indicators obtained from the catastrophizing interview.The contents and characteristics of the current definition of catastrophic belief were verified by the combined application of the catastrophic beliefs scale and the catastrophizing interview.Results:1.Define catastrophic belief as cognitive belief,including its common situation and characteristics.The initial catastrophic belief scale consists of 19 items,which are composed of four factors:self-evaluation,physical safety,evaluation from others,and property security.2.The internal consistency reliability of the catastrophic belief scale was 0.782～0.901,fractional half-reliability was 0.749～0.891,and retest reliability was 0.783～0.886.In the confirmatory factor analysis,the scale was output as four effective factors:self-evaluation,others’evaluation,physical safety and property safety.The fitting indexes were?~2/df=1.757,RMSEA=0.045,RMR=0.045,GFI=0.920,CFI=0.951,NNFI=0.935,NFI=0.894,and the results of criterion validity were significant,P<0.05.There was no significant difference in the total scores of people of different genders,ages and educational backgrounds on the catastrophization belief scale,while the score of the patient group was significantly higher than that of the control group,P<0.01,and the score of each patient group was significantly different,P<0.05.3.There was no significant difference between the patient group and the control group in terms of the topic and the number of steps in the catastrophic interview,and there were significant differences in the mean score of severity and the mean score of probability,P<0.01.The total score of the catastrophization belief scale was significantly positively correlated with the average score of the severity,the total score of the likelihood and the average score of the likelihood of the catastrophic interview,P<0.05.The probability and severity indexes of the high-subgroup of the catastrophic belief scale were significantly higher than those of the low-subgroup,P<0.05.Conclusion:1.Catastrophic belief is a universal cognitive belief,this belief is usually appears in situations such as self-evaluation,evaluation from others,body safety and property safety,people will exaggerate the possibility of the occurrence of dangerous events and the severity of the consequences.The initial catastrophic belief scale can be used to identify and present the specific content of the catastrophic belief of patients with compulsive disorder,and this scle is able to show the difference of the catastrophic belief between the patients with obsessive-compulsive disorder and the normal population.2.Catastrophic belief scale has good psychological measurement characteristics.It can show the difference of catastrophic belief between patients with anxiety,depression,obsessive-compulsive disorder and normal subjects.3.There is a consistency between the assessment results of the catastrophic belief scale and the catastrophizing interview.These two tools can be applied to the investigation ofcatastrophic belief.Two important characteristics of catastrofication belief are to exaggerate the possibility and severity of events.
