Objective:To investigate the current knowledge,beliefs and behaviors of obese patients with PCOS on disease self-management,and analyze their influencing factors,to understand the real feelings of patients with obese PCOS during treatment,and to provide a reference for patients to implement targeted psychological care.Methods:(1)Questionnaire preparation: Quoting the polycystic ovary syndrome knowledge,belief and behavior questionnaire with good reliability and validity,through literature review and expert consultation,pre-investigation and reliability and validity tests were carried out,and the final version of the self-management knowledge,belief and behavior questionnaire for obese polycystic ovary syndrome patients was compiled.Cronbach’s α coefficient,split-half reliability coefficient,content validity(CVI),and exploratory factor analysis were used to test the reliability and validity.(2)Survey and research: A convenient cluster sampling was used to conduct a questionnaire survey on obese patients with polycystic ovary syndrome in a gynecological outpatient department of a tertiary hospital in Hunan from September2021 to December 2021.The questionnaires included general information questionnaire and self-management knowledge,belief,and behavior scales for obese polycystic ovary syndrome.301 questionnaires were recovered,of which 282 were valid questionnaires.The general data of patients were descriptive statistics by frequency and composition ratio;the scores of knowledge,attitude and behavior were described and analyzed by means±standard deviation;the general data of obese polycystic ovary syndrome were compared by means of two samples or multiple samples;Pearson correlation analysis method was used to analyze the correlation among patients’ self-management knowledge,attitudes and behaviors;the influencing factors of patients’ self-management knowledge,attitudes and behaviors were analyzed by multiple linear regression.(3)Qualitative study: Using the purposeful sampling method and the principle of information saturation,face-to-face semi-structured in-depth interviews were conducted with 15 obese patients with polycystic ovary syndrome in a tertiary hospital in Hunan from September 2021.The researchers transcribed the interview recordings into text and coded them,and used the Colaizzi seven-step analysis method to analyze the results.Results:(1)The self-management knowledge,letter and row scale of obese patients with polycystic ovary syndrome had good reliability and validity: the Cronbach’s ?coefficient of the total table was 0.882,and the self-management knowledge dimension Cronbach’s ? Coefficient is 0.925,Attitude Dimension is 0.936,Behavior Dimension is0.830;Half-Confidence Coefficient Is 0.909 for Part I and 0.812 for Part II;S-CVI for Total Table S-CVI is 0.955,Self-Management Knowledge Dimension I-CVI is 0.951,Self-Management Attitude Dimension I-CVI is 0.972,and Self-Management Behavior Dimension I-CVI is 0.968;Through principal component analysis and maximum cross orthogonal rotation analysis,five common factors were extracted from the questionnaire,which conformed to the dimension design of the questionnaire and had good structural validity.(2)Self-management knowledge,belief,and behavior status and influencing factors of obese patients with polycystic ovary syndrome: the self-management knowledge score was(35.66±7.62),and the t-test and one-way variance analysis showed that the knowledge score between different ages,marital status,and strong body hair was statistically significant(P<0.01);multiple linear regression analysis showed that age,marital status,and strong body hair were the main influencing factors affecting the knowledge level of self-management in patients with obese polycystic ovary syndrome The self-management attitude score was(31.38±4.07),the attitude score between different educational levels,occupation and family history was statistically significant(P<0.05)on the t-test and univariate ANOVA(P<0.05);the multiple linear regression analysis showed that educational attainment level and occupation were the main influencing factors influencing the self-management attitude of patients with obese POLYS;the self-management behavior score was(21.98±4.92),t-test and one-way ANOVA for different ages,marital status,fertility needs,Behavioral scores between medicare types were statistically significant(P<0.05);multiple linear regression analysis showed that age and fertility needs were the main influencing factors influencing the self-management behavior of patients with obese PCOS.Knowledge and attitude(r=0.118,P<0.05),knowledge and behavior(r=0.360,P<0.01),attitude and behavior scores(r=0.156,P<0.01)of obese PCOS patients 0.01)there is a positive correlation between the three.(3)Qualitative research results: The factors affecting self-management of patients with obese polycystic ovary syndrome are lack of self-control,low level of disease cognition and self-management knowledge,difficulties in family support and time management;Environmental pressure,economic pressure and fear of health damage have an impact on patients’ lives;emotional experiences in self-management of patients with obese polycystic ovary syndrome include anxiety,worry,optimism and indifference.Conclusion:(1)Obese polycystic ovary syndrome patients have low self-management knowledge level,high attitude level,and extremely low behavior level.(2)The self-management knowledge level of obese PCOS patients is affected by three factors,age,marital status,and whether or not the body hair is prosperous;the self-management attitude level is affected by two factors,education level and occupation.The level of self-management behavior was affected by two factors: age and reproductive needs.There is a positive correlation between self-management knowledge,belief and behavior.(3)The psychological experience of self-management of patients with obese polycystic ovary syndrome includes: anxiety,worry,optimism and indifferent.Patients also have concerns about fertility issues,family and environmental stress,financial stress,and health stress.The main factors affecting patients’ self-management are lack of self-control,disease cognition level,self-management knowledge level,family support,and time management.(4)Self-management support measures for obese PCOS patients: peer supervision,Internet push of relevant knowledge,full play of the role of social support and family support,and simple management methods to improve the self-management efficiency of patients.
