Effect Of Free Position On Delivery Outcome Of Epidural Analgesia In Primipara

Objective:To explore and analyze the influence of free position on the delivery outcome of primipara with epidural labor analgesia,and further explore the practicability and safety of free position on delivery in clinical practice,so as to provide basis for the clinical application of free position in primipara with epidural labor analgesia,improve the delivery quality of primipara,and improve the delivery outcome.Methods:By convenient sampling,136 primipara were selected from December 2017 to June 2019 in a hospital in Liaocheng City,Shandong Province.They were divided into the observation group(68 primipara,delivery in free position)and the control group(68 primipara,delivery in traditional position.The observation group was given delivery analgesia+free position,and the control group was given delivery analgesia+traditional position.The first stage of labor,the second stage of labor,the third stage of labor,the amount of bleeding 2 hours after delivery and the rate of neonatal asphyxia were compared between the two groups;the incidence of abnormal fetal heart,the rate of improvement of abnormal fetal heart,the incidence of shoulder dystocia,the rate of forceps delivery,the rate of cesarean section,the rate of oxytocin use,the rate of perineum lateral section and the maternal satisfaction of delivery position were compared.All the data were analyzed by spss 20.0.Results:1.Parturient time,the amount of bleeding 2 hours and neonatal asphyxia index Compared with the control group,the observation group had a lower rate of asphyxia,a lower amount of bleeding 2 hours after delivery,a shorter time of the first/second/third stage of labor.It should be noted that the differences were significant between the two groups(P<0.05).2.Abnormal fetal heart rate and the improvement rate of abnormal fetal heart Compared with the control group,the incidence of abnormal fetal heart rate in the observation group was significantly lower(P<0.05).There was no significant difference between the two groups of the improvement rate of abnormal fetal heart(P>0.05).3.The incidence of shoulder dystocia,forceps delivery,cesarean section rate,oxytocin use rate Compared to the control group,the rate of oxytocin use and cesarean section were much lower than those in the control group.However,there was no significant difference in the incidence of shoulder dystocia and forceps delivery between the observation group and the control group(P>0.05).4.The perineum side cutting rate The perineum side cutting rate of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).5.The satisfaction of the observation group was significantly better than that of the control group(P<0.05).Conclusion:1.The application of free position in epidural labor analgesia can shorten the time of the first/second/third stage of labor,and significantly reduce the incidence of abnormal fetal heart,neonatal asphyxia,perineum side cutting,cesarean section and oxytocin use,and at the same time reduce the volume of postpartum hemorrhage within 2 hours.2.Compared with the traditional position+epidural anesthesia in the process of delivery,free position+epidural anesthesia can improve the satisfaction of the parturient,which is easily accepted and recognized by the parturient.
