Senile knee osteoarthrosis is a common senile disease,there are usually a variety of treatment methods,acupuncture is one of them.Pain and abnormal gait are the main clinical symptoms and signs.How to objectively evaluate the effect of acupuncture on gait improvement in patients with senile knee osteoarthropathy is particularly important.In addition to the routine subjective evaluation methods such as VAS pain score and JO A knee osteoarthrosis treatment effect score,this study also introduces a gait collector equipped with a three-axis accelerometer to collect acceleration signals in three directions when the human body is walking,designs gait frequency and gait stability algorithms,and calculates gait frequency and gait stability indexes.A randomized controlled trial was conducted to explore the efficacy of gait improvement and the effectiveness of the evaluation method in the treatment of senile knee osteoarthrosis.PurposeBy observing the clinical effect of acupuncture and oral ibuprofen chewing tablets in the treatment of senile knee osteoarthrosis,the specific efect of acupuncture on gait improvement of senile knee osteoarthrosis was clarified.By comparison of the results of routine evaluation and comparison of the results of gait evaluation,the scientific nature of the results of gait evaluation was identified,which can provide a method for the object evaluation of clinical efficacy of senile knee osteoarthrosis.MethodThe cases of this clinical observation were all from June 2019 to March 2020.The cases of senile knee osteoarthrosis were diagnosed in the department of acupuncture and moxibustion of Beijing mentougou hospital of TCM.According to the method of random number table,patients with senile knee osteoarthrosis were divided into drug group and acupuncture group.There was no significant difference in the course of disease,age and sex between the acupuncture group and the drug group(P>0.05).On the basis of subjective evaluation methods such as VAS pain score and JOA knee osteoarthrosis score,according to the three-axis acceleration signal collected by gait sensor,gait frequency and gait stability algorithms are designed to objectively evaluate the therapeutic effect of knee osteoarthrosis.Four weeks after treatment,the results of VAS pain score,JOA knee osteoarthrosis score,gait frequency calculation and gait stability calculation were observed in the drug group and the acupuncture group.ResultComparison of VAS pain score:there was no statistical difference in VAS pain score between the two groups before treatment(P>0.05).The VAS pain score in the acupuncture group after treatment was significantly lower than that before treatment,and the difference between the two groups was statistically significant(P<0.05).The VAS pain score in the drug group after treatment was lower than that before treatment,and the difference between the two groups was statistically significant(P<0.05).After four weeks of treatment,the VAS pain score of the acupuncture group was significantly lower than that of the drug group,and the difference between the two groups was statistically significant(P<0.05).Comparison of JOA knee osteoarthrosis score:there was no statistical difference in JOA knee osteoarthrosis score between the two groups before treatment(P>0.05).The JOA knee osteoarthrosis score in the acupuncture group after treatment was significantly higher than that before treatment,and the difference between the two groups was statistically significant(P<0.05).The curative effect score of JOA knee osteoarthrosis in the drug group after treatment was higher than that before treatment,and the difference between the two groups was statistically significant(P<0.05).After four weeks of treatment,the JOA knee osteoarthrosis score in the acupuncture group was significantly higher than that in the drug group,and the difference between the two groups was statistically significant(P<0.05).Comparison of gait frequency:there was no statistical difference in gait frequency between the two groups before treatment(P>0.05).but the gait frequency in the acupuncture group after treatment was significantly higher than that before treatment(P<0.05).In the drug group,the gait frequency after treatment was higher than that before treatment,and the difference before and after treatment was statistically significant(P<0.05).After four weeks of treatment,the gait frequency of the acupuncture group was significantly higher than that of the drug group,and the difference between the two groups was statistically significant(P<0.05).Comparison of gait stability:there was no statistical difference in gait stability between the two groups before treatment(P>0.05).The gait stability in the acupuncture group after treatment was significantly higher than that before treatment,and the difference of gait stability before and after treatment was statistically significant(P<0.05).The gait stability of the drug group after treatment was higher than that before treatment,and the difference of gait stability before and after treatment was statistically significant(P<0.05).After four weeks of treatment,the gait stability of the acupuncture group was significantly higher than that of the drug group,and the difference of the two groups was statistically significant(P<0.05).Comparison of clinical efficacy:after four weeks of treatment,52 cases were cured in the acupuncture group,the cure rate was 86.7%,and the total effective rate was 96.7%;in the drug group,40 cases were cured,the cure rate was 66.7%,and the total effective rate was 83.3%.The cure rate and total effective rate of the acupuncture group were higher than those of the drug group,and there were significant differences in the cure rate and total effective rate between the acupuncture group and the drug group(P<0.05).ConclusionThe acupuncture scheme of the acupuncture group and the ibuprofen chewing tablet scheme of the drug group were effective in improving the VAS pain score of senile knee osteoarthrosis,the curative effect score of JOA knee osteoarthrosis,gait frequency and gait stability,but the improvement effect of acupuncture scheme was significantly better than that of ibuprofen chewing tablets.At the same time,it verified the effectiveness of VAS pain score,JOA knee osteoarthrosis score,gait frequency algorithm and gait stability algorithm in the evaluation of gait improvement.
