ObjectiveStudy the changes and characteristics of gait parameters of patients with knee osteoarthritis before and after total knee arthroplasty(TKA)by using Portable Gait Analysis System Senno Gait,compare he difference from normal people,evaluate the impact on foot posture during walking by TKA and guide rehabilitation treatment after total knee arthroplasty.Methods46 patients who had operation of TKA between Jan.2017 to Oct.2017 in the orthopedics department of Qingdao Municipal Hospital were selected to measure the gait parameters 1 week before their surgery and 3 months after it by using Portable Gait Analysis System Senno Gait.At the same time,gait parameters of 20 healthy people were collected as control group to compare with and to do the prospective case-control trial analysis.Those 46 patients include 17 males and 29 females,with an average age of(70.44±8.00)years,height of(164.2+8.1)cm,weight of(68.64±14.34)kg,and body mass index(body mass index,BMI)were all less than 30kg/m2.The reasons why they were selected are as follows: all patients are end-stage varus deformity of knee osteoarthritis who did unilateral primary operation.X ray showed serious degenerative changes of the affected side knee joint.Patients with obvious narrowing or disappearance of the joint space did not have symptoms of contralateral osteoarthritis of the knee joint.Also patients should be able to walk more than 30 meters without auxiliary conditions and be able to cooperate with the examination.All of them were informed of the experiment and signed an informed agreement before they were tested.Exclusion criteria: patients with spinal,hip,ankle,or nervous system disorders affecting walking function and knee replacement.The control group consisted of 10 males and 10 females,with an average age of(67.23±7.12)years,height of(166.4±7.3)cm,body weight of(65.64±12.84)kg,and BMI(23.67±2.34)kg/m2.All healthy people should have no history of dyskinesia and pain in the hip and knee joint and wear this measuring device walking about 100 s in a straight line at a normal natural speed.The first 40 seconds allowed the subjects to fit the gait insoles and collect data after 60 s to analyze the gait parameters of the affected side before and after operation,and to compare the difference from normal person.ResultsNone of the 46 cases had complications and all the tests were completed with follow-up rate 100%.There was no significant difference in basic information between the patients and the control group.Subjective score showed that VAS score,HSS score and WOMAC score were significantly improved before and 3 months after operation.The results of gait parameter score showed that the parameters of the affected limbs such as stride length,step height,landing elevation angle,elevation angle from the ground,maximum swinging velocity,total support time on one side,swinging time on one side and support time on one foot were significantly improved 3 months after operation.Before the surgery,all of these had significant difference with normal people,but after it,most of them(stride size,step height,elevation angle,total support duration,swing time on one side and the support time of one foot)had no obvious difference with control group except the angle of elevation and the maximum wobble velocity.This had statistical significance.Conclusion1.After 3 months of TKA,the pain of knee joint was obviously improved the function was well recovered.2.The length of step,step height,landing angle,elevation angle,maximum swinging velocity,total support time on one side,swing time on one side and support time on one foot of the affected side were significantly improved compared with those before operation.The results showed that TKA had a good effect on the gait of knee osteoarthritis,the parameters before operation were significantly different from those in normal subjects,which indicated that the knee osteoarthritis had a significant effect on gait.There were still a significant difference in the individual parameters between the postoperative parameters and the normal people.It shows that although TKA helps to improve,they still need rehabilitation exercises to make gait return to normal.3.Portable Gait Analysis System Senno gait can monitor the gait characteristics of patients after TKA in real time,and objectively record the changes of gait before and after it.It has a certain reference value for evaluating the effect of surgery,and can evaluate the severity of the disease in the clinical stage,as well as stage guidance for postoperative rehabilitation,which can effectively promote the early exercise and rehabilitation of knee joint function after operation.
