【Background】The role of the knee joint in the process of lower limbs is very important.It is the largest joint in the whole body and its structure is extremely complicated.The dynamic stability of the knee joint during exercise requires a variety of internal and external joints to maintain the joint.Among them,the anterior cruciate ligament(ACL)plays a pivotal role,and it mainly restricts the excessive advancement and rotation of the tibia when the knee flexion and extension activities occur.With the advent of the national sports era,sports injuries are becoming more common.Among them,anterior cruciate ligament injury(ACL deficiency)is the most common ligament injury,and ACLD is often associated with meniscus injury.According to reports,400,000 cases of knee ACL reconstruction are performed every year in the United States [1].The total incidence of ACLD in Chinese athletes is 0.43% [2].The knee ACLD can seriously affect the stability of the joints,resulting in significant knee instability.Reduce the level of movement of the injured,and secondary to cartilage and meniscus injury,accelerated knee degeneration,followed by knee osteoarthritis.Studying knee kinematics changes after ACLD can help us understand the characteristics of ACLD knee movement more clearly and guide clinical treatment and postoperative rehabilitation.This topic is divided into the following three parts:Part I Characteristics of ACLD and Meniscal Injury Knee Joint Asynchronous Velocity.【Purpose】To investigate the changes of knee joint motion characteristics of the affected knee joint with or without meniscus injury at a specific speed when walking.【Materials and Methods】A total of 60 patients admitted to our hospital and diagnosed by arthroscopy as knee ACLD with or without meniscus injury were grouped according to different meniscal lesions.The Opti-knee system was used to detect gait before surgery to the knee experimental group.With knees as the control group,the femoral internal and external displacements,internal and external rotational displacements of the femur relative to the knee were compared between the 2 km/h(V-2,slow)and 4 km/h(V-4,fast)walkings.Flexion and extension angle,front and rear displacement,up and down displacement,inside and outside displacement of six degrees of freedom activity changes;【Result】When the knees were walking with V-2 and V-4 respectively,compared with the knees at the same speed,the gait analysis showed that the femoral varus was decreased,the anterior displacement of the tibia was increased,the range of the anterior and posterior tibias was increased,and the knee was unstable before and after Obviously,the external rotation of the femur increased,some of the experimental group had rotational instability,the femoral displacement and lower displacement of the femur relative to the tibia increased,and the knee joint flexion and extension activity decreased,P<0.05,the results were statistically significant;【Conclusion】When the knees were walking on V-2 and V-4 respectively,the knee joint movement characteristics were changed at the same speed when the knees were compared.The knee joints were unstable when the knees were walking at different speeds.Part Ⅱ ACLD and meniscus injury in the knee joint movement characteristics of different speeds.【Purpose】To study the effect of speed change on gait characteristics of knee ACLD with or without meniscal injury;【Materials and Methods】A total of 60 patients admitted to our hospital and diagnosed by arthroscopy as theknee ACLD with or without meniscus injury were grouped according to different meniscal lesions.The knees V-2 and V-4 were walked on a treadmill with Opti.The gait detection of the knee-gait gait system was performed to compare the changes in the knee movement characteristics of the ACLD at different speeds.【Result】In the 4 groups of patients,when walking at V-4 speed,the knee joints in the varus and varus,internal and external rotation,flexion and extension,front and rear,upper and lower,and internal and external six degrees of freedom increased more than the healthy side,P<0.05.Academic significance【Conclusion】Knee joint ACLD with or without meniscal injury.The knee joint stability during V-4 exercise was lower than that at pace V-2.Part Ⅲ The changes of the characteristics of ACLD and meniscal injury in the knee joint at different speeds.【Purpose】To study the change trend of knee joint motion parameters after walking under different speeds after knee joints in ACLD;【Materials and Methods】A total of 60 patients with ACL anterior crossover diagnosed in our hospital who were diagnosed as arthroscopically diagnosed as knee joints were grouped according to different meniscal lesions.The knees and knees V-2 and V-4 were walked on a treadmill.Under the speed,the relative gap movement parameters of the knee were compared,the absolute value of the speed difference was compared,and the effect of the observation speed on the movement trend of the knee joint movement characteristics was studied.【Result】In the 4 groups of patients,when walking at V-4 speed,the movement parameters of the ipsilateral knee compared with the healthy knee were reduced to a different extent thanthe pace V-2,P<0.05,the result was statistically significant,namely the pace V-4 The stability of the knee was changed at the same speed at the time of exercise,and it was lower than that at the same speed when the pace V-2 was exercised.【Conclusion】Knee ACLD with or without meniscus injury,the stability of the speed V-4 movement than the contralateral side,compared to the speed V-2 tends to reduce when moving.
