Objective:to evaluate the clinical efficacy of Yue Bi and Banxia decoction and explore its mechanism by observing the effect of Yue Bi and Banxia decoction combined with western medicine on TCM syndrome score,CAT score, inflammatory factors, blood gas analysis, lung function and other indexes in patients with phlegm-heat obstructing lung type of AECOPD,in order to seek a more safe and effective treatment scheme for phlegm-heat obstructing lung type AECOPD treated by combination of traditional Chinese and western medicine.Methods:83 patients with AECOPD of phlegm-heat obstructing the lung were randomly divided into control group(n=41)and control group(n=42).The control group was given routine treatment of western medicine, and the traditional Chinese medicine group was treated with Yue Bi and Banxia decoction on the basis of routine treatment of western medicine. Each group was treated for 10 days. After treatment, the changes of TCM syndrome scores(cough, expectoration, shortness of breath, shortness of breath),CAT score, inflammatory indexes(WBC,CRP,PCT,TNF-α,IL-6,IL-8),arterial blood gas analysis(Pa O2,Pa CO2),pulmonary function(FEV1%,FEV1/FVC)were observed and compared. After the end of the trial, the data were recorded into SPSS25.0 statistical software for data statistical analysis. To evaluate the efficacy and safety of the two groups of treatment.Results:1.General condition:there was no significant difference in sex and age between the two groups(P>0.05).2.TCM syndrome score:both groups decreased after treatment, the difference was statistically significant(P<0.01),the improvement degree of the traditional Chinese medicine group was better than that of the control group, the difference was statistically significant(P<0.05).3.The effective rate of TCM syndrome in the traditional Chinese medicine group was better than that in the control group, and the difference was statistically significant(P<0.05).4.CAT score:both groups decreased after treatment, the difference was statistically significant(P<0.01),the traditional Chinese medicine group was better than the control group, the difference was statistically significant(P<0.05).5.Inflammatory indicators:WBC,CRP,PCT,TNF-α,IL-6,IL-8 in the two groups were all lower than before, and the difference was statistically significant(P<0.01);comparison between the two groups,WBC in the Chinese medicine group The degree of improvement was better than that of the control group, and the difference was statistically significant(P<0.01).The improvement degree of CRP,PCT,TNF-α,IL-6,IL-8 and other indicators was better than that of the control group, and the difference was statistically significant(P<0.05).6.Blood gas analysis and pulmonary function:FEV1%,FEV1/FVC,Pa O2and Pa CO2 were all improved after treatment in the two groups, but there was no significant difference in the improvement of FEV1%,FEV1/FVC,Pa O2and Pa CO2 between the two groups. Conclusion:the curative effect of Yue Bi and Banxia decoction combined with western medicine in the treatment of AECOPD patients with phlegm-heat obstructing the lung is better than that of western medicine alone, and it can better improve the TCM syndrome score,CAT score and inflammatory factors, but there is no significant difference between Yue maidservant plus Banxia decoction and western medicine in improving blood gas analysis and pulmonary function. In the course of this clinical trial, there are no adverse drug reactions, and the safety is high, which is worthy of clinical application.
