Study On The Distribution Of TCM Syndromes Of Occipital Neuralgia And The Law Of Drug Use

Objective:The purpose of this study is to explore the distribution of TCM syndrome types of occipital neuralgia,reveal the clinical medication rules and provide evidence for the prevention and treatment of the disease.Methods:1.The baseline data and the data of four TCM diagnoses of 150 patients with occipital neuralgia were collected by using the patient information collection table.The factor analysis method was used to summarize the distribution of TCM syndrome types of occipital neuralgia.2.By searching the relevant literatures on the treatment of occipital neuralgia with oral Chinese medicine in the database,the prescription data in the medical cases meeting the inclusion criteria were screened out.Data mining technology and association rule analysis method were used to dig the rule of drug use.Results:1.The number of female patients is about 1.21 times that of male patients.The majority of patients are between 40 and 59 years old,accounted for 32.00% of all patients.2.The distribution of TCM syndrom types of occipital neuralgia mainly includes six types:the syndrome of wind-cold attacking the surface,the syndrome of wind-heat attacking the surface,the syndrome of obstruction of collaterals by blood stasis,the syndrome of liver-fire Ascending,the syndrome of orifices confused by phlegm,and the syndrome of kidney essence deficiency.3.By searching the database and summarizing the rules of occipital neuralgia medication find that the total usage frequency of the 132 Chinese herbs included is 508 times.There are 23 traditional Chinese medicines used more than or equal to 7 times,and the frequency of use is260 times,accounting for 51.18% of the total frequency of use.All the traditional Chinese medicines are divided into 25 categories,among which the top three most frequently used medicines are medicines for evacuate wind-cold,medicines for promoting blood circulation and removing blood stasis,and medicines for regulating liver and calming endogenous wind.4.According to the results of association rule analysis,the highfrequency drug combinations are divided into six groups,namely,the evacuating wind-cold group,the blood circulation and stasis removing group,the regulating liver and calming endogenous wind group,the evacuate wind-heat group,the clearing damp phlegm group and the replenishing kidney essence group.Conclusion:1.There is no significant difference in age composition between male and female patients.The peak age is 40-59,and the prevalence decreased significantly after the age of 60.2.The common TCM syndromes and treatment methods of occipital neuralgia are as follows:(1)The syndrome of wind-cold attacking the surface:Mainly the use of medicines for evacuating wind-cold.The representative prescriptions are Gegen Decoction and Chuanxiong Chatiao Powder,and the commonly used drug combination is Radix Puerariae–Glycyrrhiza-Notopterygium Root.(2)The syndrome of wind-heat attacking the surface:Mainly the use of medicines for evacuating wind-heat.The representative prescription is Decoction of Xiong-zhi-shi-gao.The commonly used drug combination is Herba schizonepeta-Herba schizonepeta.(3)The syndrome of obstruction of collaterals by blood stasis:Mainly the use of medicines for promoting blood circulation for removing blood stasis,and appropriate with medicines for regulating qiflowing.The representative prescription is Tongqiaohuoxue Decoction.The commonly used drug combination is Radix Paeoniae Rubra-Angelica Sinensis.(4)The syndrome of liver-fire Ascending:Mainly the use of medicines for regulating liver and calming endogenous wind,which can be combined with medicines for clearing heat-fire and dispersing stagnated liver qi.The representative prescription is Longdan Xiegan Decoction.The commonly used drug combinations are Gastrodia elata-Uncaria rhynchophylla,Scorpion-Centipede.(5)The syndrome of orifices confused by phlegm:Mainly the use of medicines for eliminating dampness and diuresis,medicines for removing heat-phlegm,medicines for removing cold-phlegm,medicines for resolving of dampness,medicines for removing heat-dampness,medicines for removing wind-cold-dampness,medicines for removing wind-heat-dampness and medicines for removing wind-dampness and strengthen bones and muscles can be used separately.Can also be combined with medicines for regulating qiflowing to resolving phlegm.The representative prescription is Banxia Baizhu Tianma decoction.The commonly used drug combinations are Radix Angelica Pubescens-Herba Taxilli,Ramulus Cinnamomi-Poria-Alisma Orientalis.(6)The syndrome of kidney essence deficiency:Mainly the use of medicines for reinforcing yang,medicines for tonifying Kidney to control nocturnal emission,medicines for nourishing yin and medicines for clearing deficient heat.The representative prescription is Liuwei Dihuang Pill.The commonly used drug combination is Eucommia ulmoides-Dipsacus asperoides.
