Observation On The Curative Effect Of Andian Ertian Decoction And Baoyin Decoction Combined With Progesterone In The Treatment Of Subchorionic Hematoma With Kidney Deficiency And Blood Heat

Objective: To study the clinical effects of Andian Ertian decoction and Baoyin decoction combined with progesterone on patients with early threatened abortion complicated with subchorionic hematoma(SCH)of renal deficiency and blood heat,and to explore the efficacy and safety of the treatment of early threatened abortion complicated with SCH by single application of western medicine or the combination of Chinese and western medicine.Methods: Fifty-eight patients who met the criteria for the diagnosis and inclusion of SCH in early threatened abortion of renal deficiency and blood heat were randomly divided into experimental group and control group.Among them,the control group was routinely given dydrogesterone tablet,twice a day,10 mg each time.Experimental group on the basis of the control group added traditional Chinese medicine decoction,prescription selected Andian Ertian decoction and Baoyin decoction plus or minus.B-ultrasonography and blood values were monitored every 1 week in both groups.If clinical symptoms and SCH disappeared under B-ultrasonography,medication was stopped.Otherwise,medication was continued for 2 weeks.Before and after treatment,TCM syndrome score,SCH area under B-ultrasound and serum progesterone(P) were recorded,the clinical efficacy of TCM syndrome and clinical comprehensive efficacy were counted,medication safety was observed,statistical software was used to analyze the data,and conclusions were drawn.Results:1.There were no significant differences between the experimental group and the control group in general conditions(age,gestation time,vaginal bleeding time,number of abortions),TCM syndrome score before treatment,SCH area under B-mode ultrasound before treatment,and serum P level before treatment(P > 0.05),indicating that the efficacy of the two groups was comparable.2.Compared with TCM syndrome scores,intra-group comparison showed that there were statistically significant differences between the two groups after treatment and before treatment(P < 0.05).After treatment,the score of the experimental group(1.79±1.17)was significantly lower than that of the control group(5.07±4.66),and the difference was statistically significant(P < 0.05).3.Compared with the SCH area under B-mode ultrasound,the SCH area in both groups was significantly reduced or disappeared after treatment,and the difference was statistically significant(P < 0.05).The reduction of SCH area in the experimental group was greater than that in the control group,and the difference between groups was statistically significant(P < 0.05).4.Compared with the control group,the serum P level increased after treatment,with statistical significance(P < 0.05).Compared with the control group,the serum P level of the experimental group was only slightly higher than that of the control group,and there was no significant difference between the two groups(P > 0.05).5.Compared with the curative effect of TCM clinical syndrome,57.14%of the patients in the experimental group were basically cured,28.57%of the patients had significant curative effect,7.14% of the patients had slight effect,and 7.14% of the patients had no treatment.In the control group,23.33% were basically cured,43.33% had significant curative effect,10.00% had slight effect,and 23.33% had no effect.The experimental group had better efficacy of TCM clinical syndromes,and the difference was statistically significant(P < 0.05).6.Compared with the comprehensive clinical efficacy,the total effective rate of the experimental group was 92.86%,and the total effective rate of the control group was 76.77%.The clinical comprehensive treatment effect of the experimental group was better than that of the control group,and the difference between them was statistically signifi-cant(P < 0.05).7.Safety comparison,the experimental group and the control group during the treatment of drugs were found no adverse reactions.Conclusion: For patients with early threatened abortion complicated with SCH of kidney deficiency and blood heat,the application of Andian Ertian decoction and Baoyin decoction combined with dydrogesterone has a good effect on improving SCH area,TCM clinical syndrome and serum P level under B-ultrasound.However,combined use of Chinese and Western medicine has a better effect on the elimination of SCH area and the alleviation of TCM clinical syndromes.
