| Purpose: This study explores the important role of lisfranc ligament in midfoot injuries under the guidance of the theory of "Tendonbundle bone" in traditional Chinese medicine,and demonstrates the relationship between tendons and bones in lisfranc injuries at the same time.The aim is to further reveal the importance of equal emphasis on muscles and bones in midfoot injuries through the theory of "Tendonbundle bone".Methods: Collected 40 cases of lisfranc injury hospitalized from October 2018 to September 2020 in the Department of Traumatology and Orthopedics of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine.According to the different ways of taking medication after surgery,they were randomly divided into two groups: experimental group(open reduction lisfranc screw or plate fixation combined with postoperative oral Jieguxujinfang)20cases and control group(open reduction lisfranc screw or plate fixation)20cases.The experimental group and the control group were routinely changed dressings after the operation,anti-inflammatory and swelling,and were given the same postoperative functional exercise guidance.The experimental group started to take Jieguxujin Decoction on the 7th day after surgery for 3 months,while the control group was not given Jieguxujin Decoction.The swelling and subsidence of the experimental group and the control group were observed after the operation,and the postoperative reexamination was performed to observe the imaging examination results of the two groups.The patient’s foot function was evaluated by the Maryland foot function scoring standard at the 3rd and6 th month after the operation,and the data was analyzed by SPSS22.0statistical software.Results: During the follow-up,there were no lost cases in the experimental group and the control group,and finally all the included cases completed the study.There was no significant difference between the experimental group and the control group in age,callus appearance time,and postoperative bone healing time(P>0.05).During the follow-up period,imaging examination showed that there was no delayed union or nonunion of fracture in either group.Three months after surgery,the Maryland foot function score showed that the highest score of the experimental group was 90 points and the lowest score was 43 points;the highest score of the control group was 85 points,and the lowest score was 48 points.There was no statistically significant difference between the two groups(P >0.05).The Maryland foot function score at 6 months after surgery showed that the highest score of the experimental group was 96 points and the lowest score was 72 points;the highest score of the control group was 93 points,and the lowest score was 43 points.The difference between the two groups was statistically significant(P<0.05),indicating that the foot function of the experimental group was better than that of the control group at 6 months postoperatively.Conclusion: 1.The theory of “Tendonbundle bone” has important guiding significance in the preoperative diagnosis of lisfrac injury,intraoperative fixation,and postoperative recovery.2.Jie Gu Xu Jin Fang can promote the postoperative recovery and foot function of patients with lisfranc injury to a certain extent. |
