| Objective:The comorbidities of hospitalized patients with psoriasis were analyzed and the risk factors were screened to provide objective basis for the prevention and early detection of psoriasis comorbidities.The distribution of TCM syndromes in hospitalized patients with psoriasis was analyzed to provide guidance for TCM treatment and early intervention of psoriasis comorbidities.Methods:Patients with psoriasis hospitalized in the Department of Dermatology of Shandong Hospital of Traditional Chinese Medicine from July2014 to July 2020 were collected and filled the Registration Form of Psoriasis Inpatients in Shandong Hospital of Traditional Chinese Medicine through the digital medical record browser.The data in the registration form were input into Excel,and SPSS26.0 statistical software was used to conduct statistical analysis on the incidence,risk factors,TCM syndromes and etiology and pathogenesis of the comorbidities.Results:1.A total of 973 hospitalized patients with psoriasis were collected in this study,including 661 males(67.9%)and 312 females(32.1%),male:female=2.12:1.The peak age of patients was 20-39 years old(47.3%).According to BSA,the severity of psoriasis was classified,and severe psoriasis(916 cases,94.1%)was the most common.2.Psoriasis vulgaris was the most common,with a total of 803 cases(82.5%).174 patients(17.9%)had a history of smoking,145 patients(14.9%)had a history of alcohol abuse,and 213 patients(21.9%)had a family history.3.In this study,a total of 475 patients(48.8%)with psoriasis had comorbidities.Hypertension had the highest prevalence rate,accounting for 14.1%(137 cases),followed by joint disease(85 cases,8.7%),hyperlipidemia(74cases,7.6%),infection(72 cases,7.4%),liver disease(71 cases,7.3%),cardiovascular disease(66 cases,6.8%),diabetes(59 cases,6.1%).In addition,it also inciuded other skin diseases,respiratory diseases,nervous system diseases,digestive tract diseases and so on.4.In this study,gender(OR:2.087,95%CI:1.190-3.658),age(OR:1.099,95%CI:1.078-1.122),arthropathic psoriasis(OR:10.090,95%CI:1.364-74.634),cardiovascular disease(OR:3.712,95%CI:1.87-7.368),hyperlipidemia(OR:2.504,95%CI:1.275-4.918)were the influencing factors of hypertension in patients with psoriasis.Age(OR:1.062,95%CI:1.038-1.086)and hypertension(OR:3.658,95%CI:1.992-6.719)were the influential factors of psoriasis patients complicated with cardiovascular disease.Age(OR: 1.092,95%CI: 1.064-1.121)and hyperlipidemia(OR:7.464,95%CI:3.846-14.484)were the influencing factors of diabetes in patients with psoriasis.Age(OR:1.029,95%CI:1.004-1.056),history of alcohol addiction(OR:7.036,95%CI:3.047-16.246)and diabetes mellitus(OR:9.081,95%CI:4.366-18.890)were the influencing factors of hyperlipidemia in patients with psoriasis.There were statistical differences in the above factors(P <0.05).5.In the psoriasis comorbidities group,the most common main syndromes were blood heat syndrome(204 cases,42.9%),qi and blood stasis syndrome(115 cases,24.2%),dampness-heat syndrome(65 cases,13.7%).Blood stasis(79 cases,27.6%)and dampness-heat syndrome(51 cases,17.8%)were the most common clip syndromes.The main syndromes of group without psoriasis comorbidity were blood heat syndrome(265 cases,53.2%),qi and blood stasis syndrome(87 cases,17.5%),and its most common clip syndrome were blood stasis(56 cases,26.4%),blood deficiency(35 cases,16.5%),wind-heat(35cases,16.5%).The main syndrome and concurrent syndromes between the two groups were statistically different(P<0.05).Conclusion:1.Psoriasis patients are easily associated with hypertension,hyperlipidemia,infection,liver disease,cardiovascular disease,diabetes.2.Gender,age,arthropathic psoriasis,cardiovascular disease and hyperlipidemia could be considered as risk factors for hypertension.Age and hypertension can be the risk factors for cardiovascular disease.Age and hyperlipidemia may be the risk factors for diabetes mellitus.Age,the history of alcoholism and diabetes mellitus may be the risk factors of hyperlipidemia.They can provide objective basis for the prevention and early detection of psoriasis comorbidity.3.Psoriasis comorbidities is mostly a mixture of deficiency and excess,which is originally a dysfunction of the functions of the viscera,and can not be separated from the participation of phlegm,dampness and blood stasis.To provide guidance for early intervention and TCM treatment of psoriasis comorbidities. |
