Clinical Observation Of Fuyuan Huoxue Decoction On The Prevention Of Lower Extremity Deep Vein Thrombosis After Direct Anterio Approach Total Hip Arthroplasty

Objective:By observation the differences of conventional treatment and the use of Fuyuan Huoxue decoction on the basis of conventional treatment to prevent the formation of lower limb deep vein thrombosis after direct anterior approach total hip replacement,it provides a safer and more effective way to prevent the formation of lower limb deep vein thrombosis after direct anterior approach total hip replacement.Methods:A total of 60 patients who underwent direct anterior approach total hip replacement due to femoral head necrosis or hip dysplasia in the first section of the Second Hospital Affiliated to Xiamen University from January 2022 to December 2022 were selected as the study subjects.All patients were randomly divided into experimental group and control group,with 30 cases in each group.The control group received conventional postoperative treatment,and the experimental group,based on the control group,began to take the oral administration of Fuyuan Huoxue Decoction on the first day after surgery,100 ml each time,once in the morning and once in the evening,for 2 weeks.In both groups,lower limb color Doppler ultrasonography was performed before surgery,1 day,15 days,and 1 month after surgery,and D-dimer,fibrinogen,Wells score,thigh circumference and calf circumference were measured and recorded before surgery,1 day,4 days,7 days,and 1 month after surgery.The obtained data were collated and analyzed statistically by SPSS 26.0.Results:1.The final completion of this subject was 30 cases in the experimental group and 30 cases in the control group.There were no significant differences in gender,age,BMI,etiology,surgical side and duration of operation between the two groups(P > 0.05),indicating that the two groups were comparable.2.Lower limb color ultrasound results: deep vein thrombosis occurred in both groups,with 3cases in the control group and 1 case in the experimental group.There was no statistical significance in the incidence of lower limb DVT between the two groups(P > 0.05).3.D-dimer: There was no statistical significance in the D-dimer value between the two groups before surgery and the 1st and 4th days after surgery(P > 0.05),while the D-dimer value of the experimental group was significantly lower than that of the control group at the 7th and 1st month after surgery,with statistical significance(P < 0.05).4.Fib: There was no significant difference in Fib values between the two groups before surgery and on the 1st and 4th day after surgery(P > 0.05),and the Fib values of the experimental group were significantly lower than those of the control group on the 7th and 1st month after surgery,with statistical significance(P < 0.05).5.Wells score: Wells scores of the two groups before surgery,1 day and 4 days after surgery had no statistical significance(P > 0.05),and Wells scores of the experimental group were significantly lower than those of the control group 7 days and 1 month after surgery,with statistical significance(P < 0.05).7.Large and calf circumference: There was no statistical significance in the large and calf circumference between the two groups before surgery and on the 1st and 4th day after surgery(P > 0.05),and the experimental group was significantly lower than the control group at the 7th and1 st month after surgery,with statistical significance(P < 0.05).Conclusion:Taking Fuyuan Huoxue decoction on the basis of conventional treatment after direct anterior approach total hip replacement can effectively improve the coagulation index of patients,reduce the incidence of DVT,conducive to early functional exercise of hip joint,in line with the concept of rapid rehabilitation,worthy of further clinical research,promotion and application.
