| Objective:The clinical observation of the effect of Jianpi Wenshen Xiezhuo Recipe on the effect of enterogenous urinary toxin p-cresol sulfate in patients with CKD3-4 spleen-kidney Yang deficiency syndrome provides a certain level for the development of the "gut-renal axis" theory in traditional Chinese medicine Clinical basis.Methods: A total of 60 patients with CKD3-4 spleen-kidney-yang deficiency in the inpatient and outpatient departments of the Nephrology Department of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from March 2020 to December 2020 were selected.30 cases in each group,the two groups were given the same basic treatment.The experimental group was treated with Jianpi Wenshen Xiezhuo Decoction,and the control group was treated with Bifidobacterium Lactobacillus triple live bacteria tablets.After 3 months of treatment,the two groups were compared before and after treatment.Changes in serum p-cresol sulfate,TCM syndrome score,Serum creatinine,Blood urea nitrogen,high-sensitivity C-reactive protein,and safety indicators(serum K,alanine aminotransferase,aspartate aminotransferase)among patients.Results:(1)In terms of p-cresol sulfate: after 3 months of treatment,comparison within the treatment group: the average value of p-cresol sulfate before treatment in the treatment group was(97.32±27.32)pg/ml,and the average value of p-cresol sulfate after treatment was(97.32±27.32)pg/ml The value was(76.45 ± 16.60)pg/ml,the difference was statistically significant(P<0.05).Comparison within the control group: the average value of p-cresol sulfate before treatment in the control group was(100.77±26.96)pg/ml,and the average value of p-cresol sulfate after treatment was(84.65±22.24)pg/ml,the difference was statistically significant(P<0.05).Comparison between the two groups after treatment: Compared with the control group,the level of p-cresol sulfate in the treatment group decreased more significantly,but the difference was not statistically significant(P>0.05).(2)In terms of blood creatinine: after 3 months of treatment,comparison within the treatment group: the average value of blood creatinine before treatment in the treatment group was(181.59 ± 65.31)ummol/l,and the average value of blood creatinine after treatment was(139.59 ± 50.46)ummol/l,the difference was statistically significant(P<0.05).Comparison within the control group: the average blood creatinine of the control group before treatment was(184.03 ± 64.06)ummol/l,and the average blood creatinine after treatment was(168.20±56.53)ummol/l,the difference was not statistically significant(P>0.05).Comparison between the two groups after treatment: Compared with the control group,the blood creatinine of the treatment group decreased more significantly,and the difference was statistically significant(P<0.05).(3)In terms of urea nitrogen: after 3 months of treatment,comparison within the treatment group: the average value of urea nitrogen before treatment in the treatment group was(11.53 ± 3.95)ummol/l,and the average value of urea nitrogen after treatment was(8.57±2.98)ummol/l,the difference was statistically significant(P<0.05).Comparison within the control group: The average value of urea nitrogen in the control group before treatment was(11.22 ± 4.52)ummol/l,and the average value of urea nitrogen after treatment was(10.46±3.74)ummol/l,but the difference was not statistically significant(P>0.05).Compared between the groups after treatment,compared with the control group,the treatment group’s urea nitrogen decreased more obviously,and the difference was statistically significant(P<0.05).(4)In terms of hypersensitivity C-reactive protein: after 3 months of treatment,comparison within the treatment group: the average value of hypersensitivity C-reactive protein before treatment in the treatment group was(6.33 ± 1.37)mg/dl,and hypersensitivity C-reaction after treatment The average protein was(4.07±2.11)mg/dl,and the difference was statistically significant(P<0.05).The average value of hypersensitivity C-reactive protein before treatment in the control group was(6.18 ± 1.51)mg/dl,and the average value of hypersensitivity C-reactive protein after treatment was(4.91 ± 1.78)mg/dl,the difference was statistically significant(P<0.05).Comparing the two groups after treatment,compared with the control group,the treatment group had a more significant decrease in hypersensitivity C-reactive protein,but the difference was not statistically significant(P>0.05).(5)In terms of TCM symptom score: after 3 months of treatment,comparison within the treatment group: Compared with before treatment,the symptom score of the treatment group was significantly reduced after treatment,and the difference was statistically significant(P<0.05).Comparison within the control group:Compared with before treatment,the symptom score of the control group was significantly reduced after treatment,and the difference was statistically significant(P<0.05).Comparison between the two groups after treatment:Compared with the control group,the TCM syndrome score of the treatment group decreased significantly,and the difference was statistically significant(P<0.05).(6)In terms of the efficacy of TCM syndromes: after 3 months of treatment,the total effective rate of the treatment group is 90%,the total effective rate of the control group is 83%,the treatment group is significantly better than the control group,the difference is statistically significant(P<0.05).(7)In terms of clinical efficacy: after 3 months of treatment,the total effective rate of the treatment group was 93%,and the total effective rate of the control group was 83%.The treatment group was significantly better than the control group,and the difference was statistically significant(P<0.05).Conclusions:(1)Jianpi Wenshen Xiezhuo Recipe can significantly reduce the levels of serum p-cresol sulfate,Serum creatinine,Blood urea nitrogen,and high-sensitivity C-reactive protein in patients with spleen-kidney yang deficiency in CKD3-4.(2)Jianpi Wenshen Xiezhuo Recipe can significantly reduce the patient’s TCM syndrome score,improve the patient’s TCM syndrome curative effect,and improve the patient’s clinical symptoms.(3)Jianpi Wenshen Xiezhuo Decoction can effectively reduce the level of intestinal urinary toxins,improve the patient’s micro-inflammatory state and clinical symptoms,and delay the progression of renal function. |
