Protective Clinical Efficacy Of Colonic Dialysis Combined With Modified Yishen Xiezhuo Formula On The Stage 3-5 Chronic Kidney Disease Based On The Theory Of Gut-Kidney Axis

Research objectives:1.To summarize the rule of using traditional Chinese medicine(TCM)in the treatment of chronic kidney disease by colonic dialysis and the effect of TCM colonic dialysis in the treatment of chronic kidney disease,and to provide theoretical basis for the clinical application of TCM colonic dialysis in the treatment of chronic kidney disease.2.Based on the theory of gut-kidney axis,the efficacy and safety of modified Yishen Xiezhuo formula colon dialysis in the treatment of stage 3-5 chronic kidney disease were discussed,which provided the basis for exploring the mechanism of modified Yishen Xiezhuo formula colon dialysis in the treatment of chronic kidney disease.Research methods:1.Literature on colon dialysis treatment of chronic kidney disease was collected through the database,literature that met the standard was screened,information of TCM,prescriptions,authors and titles were extracted,and a database was established.SPSS27.0 was used to analyze the name,category,sex,taste and meridian normalization of TCM in the database,and association analysis and cluster analysis were conducted for drugs with high frequency of use.To explore the law of drug use.2.Literature on colonic dialysis in the treatment of chronic kidney disease was collected through the database,and literature meeting the criteria of sodium chloride was screened.The clinical efficacy and safety of TCM colonic dialysis in the treatment of chronic kidney disease were studied by Meta analysis.3.Patients who met the sodium drainage criteria were screened,and their data(including demographic data,medical history data,biochemical indicators,data of four diagnoses of TCM,symptoms,etc.)were collected.Modified Yishen Xiezhuo formula colon dialysis,once every other day,for a total of 2 weeks.Renal function(serum creatinine,serum urea,serum uric acid,glomerular filtration rate),TCM syndrome score and clinical comprehensive efficacy were observed,and statistical analysis was performed to observe the changes of indicators.Research results:1.A total of 1502 articles were retrieved,106 of which were in line with the inclusion standard,and 76 drugs were used for 631 times.The drug with the highest frequency of use was rhubarb,which was used for 106 times,accounting for 16.80%of the total frequency of drugs.There were 8 drugs with frequency≧20 times,and a total of 18 high frequency Chinese medicines with frequency≧1.0%.Among them,the proportion of cold,bitter,liver meridian accounted for the most,according to the efficacy classification of heat-clearing drugs accounted for the largest proportion;Association rule analysis of TCM with frequency≧1.0%(frequency≧ 7 times)showed 27 association rules of drug pairs,45 association rules of 3 TCM groups and 20 association rules of 4 TCM groups.Rhubarb and Oyster had the highest degree of support among association rules of drug pairs(88.68%).Through cluster analysis,four significant cluster combinations are concluded;2.A total of 627 patients were included in the meta-analysis,and the baseline situation was similar.The results showed that TCM colon dialysis combined with TCM enema was superior to the control group in reducing the serum creatinine concentration and blood urea nitrogen concentration,improving glomerular filtration rate and effective rate(P<0.01),while there was no statistical significance in reducing uric acid(P>0.05);3.A total of 25 patients with CKD were included in this study,including 17 males(68%)and 8 females(32%).The mean age was 59.16±10.47 years.The longest course of disease was 98 months and the shortest was 3 months.After treatment,the main symptoms were less food and less dork,nausea and vomiting,body sleepiness,edema,dark complexion,and the scores of the secondary symptoms were sticky in the mouth and poor stool,and the total scores of the symptoms were lower than before treatment,the difference was statistically significant(P<0.01).The total effective rate of treatment was 68%.In terms of renal function indexes,UA and Ur were significantly improved before and after treatment,and the results were statistically different from those before treatment(P<0.05).Research conclusion:1.The addition of modified Yishen Xiezhuo formula prescription conforms to the law of medication and prescription composition;2.TCM colon dialysis can improve the clinical symptoms and signs of patients with CKD;3.The addition of modified Yishen Xiezhuo formula can reduce the toxin level of patients with stage 3-5 chronic kidney disease,delay the progression of renal function,and is safe and reliable.
