| Objective:Establishing the MCAO rat model to evaluate the effects of Siyin Recipe on cerebral infarction volume,neurological function and mortality in rats through animals.Meanwhile,observing the clinical efficacy of Si Yin Recipe in the treatment of CIS patients,and to evaluate the treatment of ischemic brain by the traditional Chinese medicine compound Si Yin Recipe efficacy in the acute phase of stroke.Method:1.Experimental study:54 rats were randomly divided into sham operation group,model group,and traditional Chinese medicine group.Sham operation group:the wound was sutured after the artery was separated and fed normally.Model group:Make MCAO models and raise them normally.Traditional Chinese medicine group:MCAO model was made,and 1.5 hours after cerebral ischemia,Chinese medicine was given intragastric treatment(the dose of Chinese medicine was 11.25g/kg.d,the concentration was 1g/ml,and it was administered twice a day).The differences in cerebral infarction volume,neurological deficit score and mortality of the three groups of rats on 1d,7d,and 14d were compared.2.Clinical research:68 patients were randomly assigned to the treatment group and the control group,and an open experimental study was adopted.Both groups were treated with conventional Western medicine.The treatment plan was in accordance with the"Chinese Acute Ischemic Stroke Diagnosis and Treatment Guide 2018"[10],The treatment group was combined with Siyinfang on the basis of conventional Western medicine treatment(1 dose a day,decoction and warm water,3 times a day,80ml/time).The course of treatment was 14 days.The efficacy of the two groups was evaluated through the TCM syndrome score and NIHSS score,and the difference in efficacy after treatment was compared.Results:1.Experimental research(1)TTC stainingAfter staining,it can be seen that there are white infarcts in the cerebral hemisphere of the ischemic side of MCAO rats,and the infarct range is consistent with the area dominated by the middle artery.The infarct rate of image analysis is about 55.28%.Compared with the sham operation group,the infarct volume of the model group and the traditional Chinese medicine group was significantly increased,and the infarct volume was significantly smaller than that of the model group after 14 days of treatment with traditional Chinese medicine.(2)Nerve function scores of rats in each groupSuccessfully modeled MCAO rats will have obvious symptoms of neurological deficits,and the scores are mostly 2 to 3 points,while the sham operation group is always 0 points.The scores of the traditional Chinese medicine group and the model group were significantly higher than those of the sham operation group,and the scores of the traditional Chinese medicine group after treatment(7 days and 14 days)were significantly lower than those of the model group.(3)Mortality of rats in each groupStatistics were performed after the experiment was completed.Compared with the sham operation group(0%),the mortality rate of the model group(61.11%)and the traditional Chinese medicine group(27.78.%)was significantly increased(P<0.01),and the mortality rate of the traditional Chinese medicine group was significantly higher than that of the model group.Decrease(P<0.05).2.Clinical research(1)The curative effect of TCM symptoms After treatment,the composition ratio of the curative effect of the two groups of patients was significantly different.The ratio of cured and markedly effective patients in the treatment group was significantly higher than that of the control group(P<0.05).The results showed that the combined treatment of traditional Chinese and Western medicine is better than pure Western medicine in improving the symptoms of CIS patients.Treatment,especially in terms of"squeaky speech,numb limbs,dizziness,and excessive phlegm",significantly improves the cure rate of patients.(2)Clinical efficacyAfter treatment,the neurological function improvement effect grade composition ratio of the two groups of patients was significantly different.The proportion of patients in the treatment group who were basically cured and significantly improved was significantly higher than that of the control group(P<0.05).The results showed that the combined treatment of Chinese and Western medicine can restore the neurological function of CIS patients The curative effect is better than pure western medicine treatment,which obviously improves the cure rate and the remarkable progress rate of patients.(3)Number of days in hospitalThe average length of stay in the traditional Chinese medicine combined western medicine treatment group was 12.00±2.11 days,which was 1.07 days less than the pure western medicine treatment group(P<0.05).(4)Safety assessmentDuring the observation period,there were no serious adverse reactions in the two groups of patients,suggesting that the four-factor prescription is safer.Conclusion:1.Experimental studies have found that:Siyin Recipe has neuroprotective effects on ischemic brain injury,significantly reduces the cerebral infarct volume of MCAO rats,improves the neurological function and dyskinesia of rats,and reduces the mortality rate of rats in the early stage of onset.2.Clinical studies have found that:Siyinfang combined with Western medicine treatment can significantly improve the TCM symptoms of CIS patients,promote the recovery of CIS patients’neurological function,increase the cure rate,and shorten the patient’s hospital stay,with better safety. |
