Objective:The study aimed to describe the status of family dynamics,rumination and post-traumatic growth(PTG)in patients with lung cancer.Explore the effects of general data on family dynamics,rumination and PTG.Analyze the correlation between family dynamics,rumination and PTG,and explore the mediating effect of rumination in family dynamics and PTG.Methods:This study is a cross-sectional survey.From September 2019 to November 2020,convenience sampling was used to sample 254 lung cancer patients from the thoracic surgery department of a tertiary hospital in Changchun City,Jilin Province.The survey was conducted by using the general information questionnaire,post-traumatic growth questionnaire,the Chinese version of the event-related rumination questionnaire,and the revised version of the System Family Dynamics Self-Rating Scale.Excel and SPSS 24.0 were used for data entry and data analysis.The statistical methods used include: descriptive analysis,independent sample t-test,one-way analysis of variance,correlation analysis,multiple linear regression analysis,Bootstrap method and so on.Results:(1)The total score of family dynamics in patients with lung cancer was(48.92±6.56),and the scores of each dimension were: family atmosphere(12.33±2.79),system logic(15.17±4.80),and individualization(10.96±3.05),Disease concept(10.54±2.02),total rumination score(29.00±1.69),invasive rumination(12.20±2.36),and purposeful rumination(16.85±3.45),The total score of PTG is(58.88±10.34),and the scores of each dimension were: relationship with others(9.91±1.92),personal strength(10.10±1.97),self-transformation(10.56±2.69),life perception(17.63±3.08),new possibility(10.69±3.11).(2)The results of univariate analysis showed that the family dynamics of lung cancer patients had statistically significant differences in educational level,personality type,and residence group scores(P<0.05).There were statistically significant differences in the level of invasive and purposeful rumination among lung cancer patients in age,gender,education level,personality type,and family income grouping scores(P<0.05).The PTG level had statistically significant differences in age,gender,education level,personality type,family income,and religious belief grouping scores(P<0.05).(3)The results of multivariate stepwise analysis show that education level and family residence enter the regression equation,explaining 66.3% of the total variation of family dynamics;gender and education level enter the regression equation,explaining 39.2% of the total variation of invasive rumination;gender and education level enter the regression equation,explaining 46.8% of the total variation of purposeful rumination;age,education level and family income entered the regression equation to explain 48.2% of the total variation of PTG.(4)There was a significant positive correlation between family dynamics and invasive rumination in lung cancer patients(P<0.01),and a significant negative correlation between targeted rumination and PTG(P<0.01).There was a significant negative correlation between invasive rumination and PTG in lung cancer patients(P<0.01),and a significant positive correlation between purposeful rumination and PTG(P<0.01).(5)Rumination produces a partial mediating effect between family dynamics and PTG.The mediating effect is-0.6924.Conclusion:Family dynamics and PTG of patients with lung cancer are in the medium level;rumination is in the lower middle level.General demographic variables have a certain impact on family dynamics,rumination and PTG.There is a correlation between family dynamics,rumination and PTG in patients with lung cancer.Rumination plays a part of the mediating effect between family dynamics and PTG.
