Objective: to analyze the expression and significance of serum interleukin(interleukin,IL)-17,IL-23 and IL-35 in patients with chronic urticaria((CU)),and to observe the efficacy of acupoint catgut embedding combined with conventional antihistamines in the treatment of CU.The purpose of this study is to explore whether catgut embedding at acupoint can regulate the immune balance of Th17/Treg cells by detecting the difference of the expression of IL-17,IL-23 and IL-35 in serum before and after catgut embedding treatment.Methods: A total of 82 patients who were diagnosed as CU in the Department of Dermatology of the first affiliated Hospital of Dali University from July 2020 to October 2020 were collected as the case group,and the peripheral blood was collected.Another 20 healthy persons who went to the physical examination center of our hospital from September 2020 to October 2020 were collected as the control group,excluding allergic diseases,autoimmune diseases and systemic diseases,sex and age matched with the case group.Double antibody sandwich method was used to detect the expression levels of IL-17,IL-23 and IL-35 in serum of patients with CU and healthy persons,and to analyze the expression level and clinical significance of IL-17,IL-23 and IL-35 in patients with CU.Eighty-two patients in the case group were randomly divided into the experimental group and the drug group.The experimental group was treated with acupoint catgut embedding plus antihistamines and the drug group was treated with antihistamines.The trial period was 3 months.After treatment,the clinical symptom and sign score scale,quality of life assessment questionnaire for patients with chronic urticaria(CU-Q2 o L)and urticaria control test were compared between the two groups before and after treatment.(UCT),took the clinical symptoms and signs score scale of urticaria as the main curative effect evaluation index to evaluate the effectiveness of the two groups.The rates of recovery,marked effect,improvement and inefficacy were statistically compared between the two groups.All the data collected were analyzed by SPSS26.0.Results:1.The total score of clinical symptoms and signs,CU-Q2 o L and UCT in the experimental group were significantly higher than those in the drug group.2.The effective rate of the experimental group was significantly higher than that of the drug group(P < 0.05).3.The serum levels of IL-17,IL-23 and IL-35 in patients with CU were significantly higher than those in healthy controls.4.There was a positive correlation between the expression of IL-17 and IL-23 in serum of patients with CU,and between the expression of IL-23 and IL-35.5.The level of serum IL-23 in the experimental group after treatment was significantly lower than that before treatment.Conclusion:1.The effect of acupoint catgut embedding combined with antihistamines in the treatment of CU is better than that of drugs alone.2.The increased levels of IL-17,IL-23 and IL-35 in patients with CU suggest that they may be involved in the pathogenesis of CU,but the relationship between the imbalance of Th17/Treg cellular immunity and the pathogenesis of CU needs further study.3.Catgut embedding at acupoints may play an immunomodulatory role by reducing the level of IL-23 in patients with CU.4.The increase of total Ig E is common in patients with CU,and the detection of total Ig E is helpful for the follow-up treatment of CU.5.The etiology of CU is complex.Before treatment,patients should be carefully asked about the suspicious cause and eradicate the cause as far as possible,which can improve the therapeutic effect and reduce recurrence.6.Sleep disorders and mental factors have a great influence on the daily life of patients with CU,so while treating the disease itself,we also need to pay attention to the mental health of patients.7.The positive rates of thyroid autoantibodies and antinuclear antibodies in patients with CU are low,but the patients with positive results of thyroid autoantibodies need longer observation and follow-up.8.The detection of allergen is not recommended as a routine examination of CU. |
