Objective:Through observation, comparison, analysis, joint thorn collaterals acupuncture cupping method with conventional acupuncture method in the treatment of cervical spondylosis of nerve root type(qi and blood stasis)the clinical efficacy of differences, investigate the clinical efficacy of acupuncture combined stab winding cupping method of value, the more effective for exploring research on acupuncture treatment of cervical spondylosis treatment,in order to improve the clinical curative effect.Method:Seventy-two patients who met the inclusion criteria were randomly divided into the experimental group(acupuncture combined with collateral acupuncture cupping group)and the control group(conventional acupuncture group)with 36 cases in each.Acupoint selection in the experimental group: Neck Jiaji,Fengchi,Dazhui,Jianjing,Tianzong,Quchi,Shusanli,Hegu,Waiguan and Houxi;Prick cupping acupoint selection: new acupoint,a is acupoint;Acupoint selection in the control group included: Neck Jiaji,Fengchi,Dazhui,Jianjing,Tianzong,Quchi,Shusanli,Hegu,Waiguan and Houxi. The two groups were treated with acupuncture once a day,5 times a week,10 times as a course of treatment,a total of 2 courses of treatment;Collateralpuncture cupping therapy was performed 2 times a week,4 times as a course of treatment,a total of 2 courses of treatment. Before and after treatment, patient case reports, observation indexes and efficacy evaluation were recorded, and the safety and efficacy of the two treatment methods were comprehensively evaluated and analyzed.SPSS26.0 was used for statistical analysis and comparison of all data before and after treatment in the two groups.Result:(1)A total of 70 cases were actually completed, including 36 cases in the experimental group(acupuncture combined with collateral acupuncture cupping group)and 34 cases in the control group(conventional acupuncture group),with 2 cases falling off. There was no significant difference in pre-treatment data(gender, age, course of disease and efficacy evaluation index)between the two groups(P > 0.05),which could be compared.(2)VAS score comparison: VAS score of the two groups after the treatment was significantly lower than that before treatment, and there was a significant difference within the group(P < 0.01).After treatment, the VAS score of the experimental group(2.75±0.69)was lower than that of the control group(3.15±0.78),and the comparison between groups was(P < 0.05).The results showed that both methods could relieve pain symptoms in patients with CSR,but acupuncture combined with collateral-pricking cupping had a better effect.(3)Comparison of NDI scores: The comparison of NDI scores between the experimental group and the control group before and after treatment(P < 0.01)indicated that both treatments could significantly improve patients’ cervical dysfunction. The comparison of NDI scores between the two groups after treatment showed a statistically significant difference(P < 0.05).The results indicated that the combination of acupuncture and cupping therapy could improve the effect of cervical dysfunction on patients’ daily life more than acupuncture alone.(4)Comparison of scores of the symptom scale of cervical spondylosis by 20points: the scores of the two groups before and after treatment were compared(P < 0.01),and the scores of the two groups after treatment were compared(P < 0.05),showing a significant difference. It indicated that the two therapies had significant efficacy in improving pain and numbness, relieving sensation, muscle strength and other physical signs, and improving work and life ability in patients with CSR.However, the improvement of scores in the experimental group was significantly greater than that in the control group, indicating that the experimental group had better efficacy.(5)Comprehensive efficacy evaluation: The total effective rate of the two groups was compared between the experimental group(94.44%)and the > control group(85.29%),Z=-0.69,(P < 0.05),showing a statistically significant difference. The results showed that acupuncture combined with collateral-pricking cupping in the treatment of cervical spondylotic radiculopathy(syndrome of qi stagnation and blood stasis)was more effective than conventional acupuncture and combined therapy.(6)Safety evaluation: Patients had no special discomfort in the treatment process, and the overall treatment process was relatively safe.Few patients had needle stagnation and subcutaneous hematoma, and no adverse events such as needle sickness and needle breaking occurred. The treatment could be continued after conventional treatment. Conclusion:Through literature review and research,we found that cervical spondylotic radiculopathy belongs to Chinese medicine "arthralgia syndrome",which mainly refers to the pain, numbness and poor flexion and extension of limbs and joints.Bi-meridian obstruction,qi-stagnation and blood stasis are the important pathogenesis and run through the whole course of the disease.Acupuncture in traditional Chinese medicine has the function of regulating qi and blood, relieving meridian and relieving acute twonus.Collateral-pricking cupping has the function of removing blood stasis, clearing away evil blood, and dredging the local meridian qi and blood.Through clinical observation and research,it is found that acupuncture combined with collateral-pricking cupping therapy is significantly better than conventional acupuncture therapy in alleviating patients’ neck pain symptoms, improving cervical spine dysfunction and improving patients’ quality of life.Especially in pain relief, some patients may have an "immediate effect".This effect may be related to the improvement of blood microcirculation, elimination of edema, inflammation and other nerve root reactions in cervical patients,as well as improvement of nerve root ischemia and hypoxia. However, further clinical experimental studies on objective indicators are still needed to prove this effect. In terms of safety observation, there were no obvious adverse reactions and side effects during the whole treatment process.Acupuncture combined with collateral-prick cupping method can firstly relieve the neck discomfort symptoms of patients with cervical spondylosis.Secondly, from the perspective of "qi stagnation and blood stasis",this therapy fully embodies the overall concept of traditional Chinese medicine and can fundamentally improve the state of qi stagnation and blood stasis. The whole treatment process runs through the core idea of the whole and local joint treatment, dialectical treatment for disease differentiation, looking for the optimal combination of combined treatment, and exploring a new diagnosis and treatment idea and new treatment method for other syndrome types and types of cervical spondylosis in many acupuncture treatment methods. |
