ObjectiveAnalyse the plasma metabolites of acute cerebral infarction(ACI)patients,and compare these markers between the large artery atherosclerosis(LAA)and the penetrating artery disease(PAD)of Chinese ischemic stroke subclassification(CISS)to more accurately diagnose or prevent the two types of ACI.MethodsWe retrospectively included 131 medical examiners from our Health Management Center and 218 non-acute cerebral infarction(NACI)patients hospitalized in the neurology department as controls,and 344 ACI patients admitted to the neurology ward as the research objects,which dated the same time.We multivariately counted the 20 kinds amino acids of human protein,citrulline,carnitine,33 acylcarnitines and 34 corresponding ratios of the ACI patients and medical examiners,and screened out valuable markers through ROC and Logistic regression.Then we removed the markers affected by between-group factors.By comparing among the physical examination group,ACI group and the NACI group,we estimated their predictive or diagnostic values.At last,we studied its characteristics between the LAA and the PAD of ACI.ResultsWe screened out the glutamate(Glu),citrulline to arginine(Cit/Arg),methionine to phenylalanine(Met/Phe),propionyl carnitine to acetyl carnitine(C3/C2)and acylcarnitine(C6)as potential biomarkers: the variable importance for the projection values were all > 1.0;the areas under ROC were all > 0.7;the P values of logistic regression analysis were all <0.05.The expression levels of these 5 indicators were higher in the ACI and NACI group than in the physical examination group,and the P values ??between the groups were all <0.05,which were statistically significant.In the LAA,the average levels of the Glu and Cit/Arg were relatively higher;while in the PAD,the Met/Phe,C3/C2 and C6 were higher.ConclusionsGlu,Cit/Arg,Met/Phe,C3/C2 and C6 may be predictive risk factors for cerebral infarction.The five indicators may all act on the LAA and PAD,which Glu,Cit/Arg may be more involved in the LAA,and the role play of Met/Phe,C3/C2 and C6 may be more in PAD.To research the process of their metabolism may provide more accurate prevention of cerebral infarction diseases.
