| Part Ⅰ Study on serum markers of acute myocardial Infarction based on metabonomicsPurpose By using the method of targeted metabonomics,the dynamic changes of amino acid metabolism profile in acute phase of acute myocardial infarction(AMI)were analyzed by liquid chromatography-mass spectrometry(LC-MS)in order to find new serum biomarkers of AMI.Methods Include 46 patients with AMI who underwent emergency PCI admitted to the Department of Cardiology,Cardiovascular Hospital Affiliated to Xiamen University,and 10 healthy volunteers were selected as the control group.The serum samples of the selected cases were separated by chromatography and analyzed by mass spectrometry.SPSS 22.0 statistical analysis software was used for t-test.The dynamic change trend charts of amino acids,hs-TNT and NT-proBNP were drawn for comparison,and the differential metabolites were screened.Results At 2h,4h,6h,12h,24h,48h and 72h after infarction,Analysis by detecting found seven amino acids having a significant difference between AMI group and control group(P<0.05).Histidine,serine,alanine,taurine and threonine were up-regulated,while methionine and tryptophan were down regulated.The changes of amino acids in 72h were not the same,For example,the increase time of alanine and threonine was earlier than hs-TNT,and the increase lasted for a long time,which is expected to be a new biomarker of AMI.Conclusion Using metabonomics analysis method,dynamic comparison of amino acid serum profile changes between AMI group and healthy control group,found that:1.There are significant differences in amino acid metabolism between the two groups;2.There are"up regulation" and "down regulation" changes in serum amino acids in AMI group,and the specific metabolic pathway needs further study;3.Some amino acids rise early and last for a long time(such as alanine and threonine).The curve is similar to hs-TNT,which may be a biomarker for the diagnosis of AMI in the future.Part Ⅱ Study on cardiovascular events in acute myocardial infarction based on metabonomicsPurpose To identify metabolic markers associated with MACE,a non-target metabonomics study using liquid chromatography-mass spectrometry(LC-MS)was used to analyze metabonomics by following MACE in patients with AMI for 1 year after PCI.Methods Include 46 patients with AMI who underwent emergency PCI admitted to the Department of Cardiology,Cardiovascular Hospital Affiliated to Xiamen University.After 1-year follow-up,they were divided into MACE group and no MACE group according to whether MACE occurred.The serum samples of the selected patients were analyzed by non-target analysis.The data were collected and preprocessed.PCA and OPLS-DA were performed by SIMCA software(v16.0.2).The disease differentiation model was established to screen different metabolites.Logistic regression analysis was performed.ROC curve were used to identify the diagnostic efficacy of different metabolites for MACE in AMI.Results Through PCA,OPLS-DA and other big data processing methods,it was found that there were significant differences in serum metabolic profiles between the AMI MACE group and non MACE group.14 metabolites were found,namely SM(d18:0/18:1(9z)),Sm(d18:1/20:0),Sm(d18:1/22:0),Sm(d18:1/16:0),Sm(d17:1/24:1(15z)),PC(22:6(4Z,7z,10z,13z,16Z,19Z)/22:5(4Z,7Z,10Z,13Z,16Z))、PI(20:2(11Z,14Z)/16:0)、N-Acetylhistidine、N6-Methyladenosine、Kynurenic acid、3-Hydroxyisovalerylcarnitine、N1-Methyl-4-pyridone-3-Carboxamide、N-a-Acetyl-L-arginine、Ubiquinone-1.And it is an independent risk factor for MACE.These metabolites were related to amino acid metabolism,phospholipid metabolism,aerobic energy metabolism,nicotinamide metabolism and N6-Methyladenosine metabolism.Conclusion Metabonomics analysis showed that 14 metabolites were associated with MACE in AMI patients after PCI.Some metabolites have high sensitivity and specificity,which could be used as potential biomarkers related to MACE for further study. |
