Purpose:Based on the previous treatment experience,the author selected the method of floating acupuncture at Ashi point to treat temporomandibular joint disorders,and carried out a detailed follow-up record of the clinical feedback during the whole treatment process,and clarified whether the method is a kind of definite curative effect and targeted It has strong sex and comprehensive effect,which is suitable for most TMD patients and conforms to the treatment methods of evidence-based medicine.Provide new methods and ideas for the clinical treatment of TMD,so that the pain of TMD patients can be reduced or eliminated,and it can greatly increase the quality of life of TMD patients.Methods: This project collected the visits and referrals of the Department of Acupuncture and Moxibustion of Hubei Provincial Hospital of Traditional Chinese Medicine from October 2017 to August 2019.A total of 68 TMD patients who met the project inclusion criteria were collected.These 68 patients were randomly divided into floating needle Ashi point.There were 34 cases in each group and the conventional acupuncture group.Among them,2cases fell off in the Ashi point group of floating needling,and 32 cases were finally included;3 cases fell off in the conventional acupuncture group,and 31 cases were finally included.The enrolled patients started treatment after receiving comprehensive and detailed health education.Take Ashi,Hegu,Xiaguan,Cheekche,Tinggong,Yifeng and other acupoints.In the treatment of the floating Ashi point group,the Ashi point is selected first,that is,2 to 3points with the most obvious tenderness on the affected side,and floating needle acupuncture.The remaining 4 to 6 acupuncture points are selected according to the patient’s specific symptoms for conventional acupuncture treatment.,The treatment is to replenish,relieve,and relieve,mainly to relieve the needle,stay the needle for 25 minutes;in the conventional acupuncture group,select 4 to 6 of the above-mentioned serious meridian points according to the clinical symptoms,and keep the needle for 25 min.The treatment frequency is one treatment every day,3 times/week,2 weeks as1 course,3 courses in total.Records during treatment:(1)Friction index is used to evaluate TMJ(temporomandibular joint,TMJ)related functions;(2)Simplified Mc Gill pain questionnaire(short-form Mc Gill pain questionnaire,SF-MPQ);among them,different time records are required Point VAS(visual analogue scale,VAS)pain score(before treatment,treatment 5 times,10 times,15 times after);(3)Maximum active Mouth Opening(MMO);(4)overall after treatment Efficacy;(5)3 months after the end of treatment,the patients participating in this experiment will be followed up or re-diagnosed,and the number of relapsed patients will be counted.Result:1.Fracton index: Before treatment,there was no significant difference in Fracton index between the two groups of patients(P> 0.05);After 3 cycles of treatment,the two datasets were compared with the corresponding pre-treatment data in each group.In contrast,the seven indices were reduced for the two patient groups after treatment,and the difference was very significant(P <0.01);After 3 cycles of treatment,the two groups of patients had P> 0.05 on the three components of JN,JP and MM and there were no statistically significant differences while P <0.05 in DI,MP,PI and CMI the difference was significant.2.SF-MPQ score: There was no significant difference in the scores of the two groups before treatment(P> 0.05).This was not statistically significant.The scores of the two groups decreased significantly after treatment,and the difference was statistically reduced.Significant(P <0.01);In the comparison between the two groups after treatment,the total score of the patient’s sensory item + emotion item and the total score of the PRI value were statistically significant,and the difference was very significant.We showed that(P = 0.000)and there was no difference between the VAS score and the PPI score.Statistical significance(P> 0.05).The VAS score also recorded the VAS score immediately after the 5th,10 th,and 15 th treatments,and the VAS scores recorded at different time points showed a significantly negative correlation with the increase in the number of treatments.The number of treatments that caused pain decreased(P <0.05);from the 5th treatment to the 15 th treatment,the pain score tended to decrease(P <0.05);but 15 times After treatment and the end of treatment,the final pain score statistics were as follows: There is no further significant change(P> 0.05).There is no significant difference in VAS score between the two groups after 5 treatments(P> 0.05),and the results are not statistically significant.However,statistical results showed that the floating needle manipulation group improved pain more significantly after the end of the 10 th treatment(P <0.05).3.MMO: The MMOs of the two groups of patients were improved before and after treatment,and the difference was statistically significant(P <0.05).However,the number of patients in the largest open group(3.5-3.9 cm)in the floating puncture group was higher than that in the conventional group.After 3courses of treatment,there was no significant difference in MMO between the two groups(P>0.05).4.Overall clinical efficacy: After 3 courses of treatment,the overall effective rate of the floating acupuncture group was 96.8%;The overall effective rate of the conventional acupuncture group is 80.6%,p <0.05 with the x2 test,and The difference in effectiveness between the 2 groups was statistically significant.5.3-month follow-up results: the overall effective rate for the floating acupuncture group was 88.4% and the conventional acupuncture group 68.1%,and the chi-square test showed that the difference between the two groups was statistically significant(P <0.05).Conclusion: Floating Ashi point manipulation and conventional acupuncture have clear clinical effects on TMD,and both can improve the three main symptoms of joint pain,snapping and restricted mouth opening.Through the analysis of the various indicators in the Fricton index,it can be understood that after 3 courses of treatment,the floating acupuncture at Ashi point is more effective in reducing the temporomandibular joint disorder index,dysfunction and improving muscle tenderness in the joint area.The comparison of the results of MM,JN,and JP between the two groups of patients shows that the effect of floating acupuncture at Ashi point is basically close to that of conventional acupuncture in improving the movement of the mandibular joint and reducing joint murmurs;the statistical analysis of the SF-MPQ score can be used.It is found that both the floating acupuncture technique group and the conventional acupuncture group can effectively reduce the pain degree of TMD patients,but the improvement of the floating acupuncture group is better,and the subjective negative emotion of the patients during the treatment period is relieved by this acupuncture method better than the conventional group;According to the statistical analysis of the results of recording the VAS scores at different time points,it can be seen that the floating acupuncture technique group showed a more obvious improvement in pain after the 10 th treatment,indicating that the floating acupuncture technique group is more effective in relieving the pain of TMD patients The conventional acupuncture group is faster.According to the results of the follow-up,it can also be shown that the long-term effect of floating acupuncture at Ashi point is better than that of conventional acupuncture,and the recurrence rate is low.In summary,the treatment of temporomandibular joint disorders by floating acupuncture at Ashi point is a method that has not yet been used as the best treatment method,but it is clearly effective and the total effective rate of treatment is higher than that of ordinary acupuncture methods.It is worthy of further study and in-depth study.analysis.
