| Objective: Chronic urticaria is a common clinical allergic skin disease.The pathogenesis is still unclear.Antihistamines as the first-choice treatment can not cure the disease.The rapid recurrence and the unbearable pruritus can bring great consequences to patients.Therefore,the prevention and treatment of chronic urticaria is the focus of modern research.Traditional Chinese medicine has rich clinical experience and successful cases in the prevention and treatment of chronic urticaria.This study observes indicators of the clinical symptom score,skin quality of life score,bad mood score,clinical efficacy score,and study the difference and characteristics of curative effect for the combination of electroacupuncture and ginger moxibustion in the treatment of chronic urticaria of wind-cold type.It is designed to provide reference value for clinical treatmentMethods: From June 1,2019 to January 31,2021,60 patients who met the clinical diagnostic criteria in the Department of Acupuncture and Moxibustion of Wuhan First Hospital were randomly divided into 2 groups of 30 patients by random number table method,namely electroacupuncture combined with ginger moxibustion group and western medicine group.The electroacupuncture group combined with ginger moxibustion group received self-made acupoints combined with ginger moxibustion at Shenque point,the western medicine group was treated with oral loratadine,and the electroacupuncture combined with ginger moxibustion group received16 treatments for 4 weeks.The first 2 weeks are treated 5 times a week,2 days off on weekends,3 times a week for the next 2 weeks,once every other day,and 2 days off on weekends.In the western medicine group,loratadine was taken orally once a day,10 mg each time.Before treatment,2 weeks of treatment,4 weeks of treatment,and 4 weeks of follow-up,clinical symptom scores,skin quality of life scores,and negative mood scores were performed,including the anxiety self-rating scale and the depression self-rating scale.Safety evaluation was performed after 4 weeks of treatment.Using SPSS25.0 statistical software,by comparing the changes in the clinical data of the patients,the clinical efficacy of the two groups of patients was analyzed and compared.Results:1.By comparing the gender,age,course of disease and the scores of clinical symptoms of urticaria,skin quality of life scores,bad mood scores and other indicators before treatment,there was no statistically significant difference between the two groups(P>0.05),and they were comparable.2.Comparison of clinical symptom scores: intra-group comparison: total clinical symptom scores: Compared with before treatment,the total clinical symptom scores of the two groups after treatment for 2 weeks and 4 weeks were significantly reduced(P<0.05);compared with 2 weeks of treatment,the total score of clinical symptoms in the electroacupuncture combined with ginger moxibustion group was significantly reduced after 4 weeks of treatment(P<0.05),while the western medicine group did not show a significant reduction(P>0.05);compared with 4 weeks of treatment,The total score of clinical symptoms in the electroacupuncture combined with ginger moxibustion group was significantly decreased during 4 weeks of follow-up(P<0.05),while the western medicine group did not show a significant decrease in4 weeks of follow-up(P>0.05).Clinical scores: Compared with before treatment,the electroacupuncture combined with ginger moxibustion group has scores of all clinical symptoms(number of wind clusters,wind cluster diameter,duration,frequency,degree of itching)after 2 weeks and 4 weeks of treatment.Significant decrease.Only the duration score of the western medicine group decreased significantly after 2 weeks of treatment(P<0.05),and the scores of other symptoms showed no statistical difference(P>0.05).After 4 weeks of treatment,The scores of other symptoms decreased significantly(P<0.05)except for the frequency.Compared with 2 weeks of treatment,the scores of clinical symptoms and symptoms in the electroacupuncture combined with ginger moxibustion group were significantly reduced after 4 weeks of treatment(P<0.05),while the western medicine group showed no statistical difference except for the number of wind clusters(P>0.05).Compared with 4 weeks of treatment,the scores of clinical symptoms in the electroacupuncture combined with ginger moxibustion group after 4 weeks of follow-up decreased significantly(P<0.05).There was no significant difference in the scores of clinical symptoms in the western medicine group(P>0.05).Comparison between groups: Compared with the western medicine group,the electroacupuncture combined with ginger moxibustion group had no significant difference in the total score of clinical symptoms,the number of wind masses,the diameter of wind masses,the duration,and the degree of pruritus after 2 weeks of treatment(P>0.05)),only the weekly seizure frequency score decreased significantly(P<0.05),indicating that the effect of electroacupuncture combined with ginger-separated moxibustion in reducing seizure frequency in a short-term course of treatment is better than that of western medicine.After 4 weeks of treatment,the total score of clinical symptoms and the number of wind masses,duration,and weekly frequency scores were significantly reduced(P<0.05),but there was no difference in wind mass diameter and itching degree scores(P>0.05).After 4 weeks of follow-up,the total scores of clinical symptoms and the scores of various symptoms in the electroacupuncture combined with ginger moxibustion group were lower than those in the western medicine group(P<0.05).3.Skin quality of life score: Comparison within the group:Compared with before treatment,the skin quality of life scores of the two groups after treatment for 2 weeks and 4 weeks of treatment decreased significantly(P<0.05);compared with 2 weeks of treatment,the two groups were in the quality of life score still decreased significantly after 4 weeks of treatment(P<0.05).Compared with the treatment for 4 weeks,the quality of life score of patients in the electroacupuncture combined with ginger moxibustion group was significantly reduced during the 4 weeks follow-up(P<0.05),while the western medicine group showed no statistical difference(P>0.05).Comparison between groups: There was no significant difference in the efficacy between the two groups after 2 weeks of treatment(P>0.05).After 4 weeks of treatment and4 weeks of follow-up,the skin quality of life scores of patients with electroacupuncture combined with ginger moxibustion decreased significantly(P<0.05),the effect is better than the western medicine group.4.Comparison of bad mood scores: Compared with before treatment,the SAS and SDS scores of the two groups after 2 weeks and 4 weeks of treatment decreased significantly(P<0.05);compared with 2weeks of treatment,the SAS and SDS scores of the two groups were still significant decreased(P<0.05).Compared with 4 weeks of treatment,the SAS and SDS scores of the patients in the electroacupuncture combined with ginger moxibustion group were significantly decreased during the 4 weeks follow-up(P<0.05),while the western medicine group showed no statistical difference(P>0.05).Comparison between groups: The SAS and SDS scores of the electroacupuncture combined with ginger moxibustion group after 2weeks of treatment were not statistically different from those of the western medicine group(P>0.05).With the increase in the course of treatment,after 4 weeks of treatment and 4 weeks of follow-up,the SAS and SDS scores of patients in electroacupuncture combined with ginger moxibustion decreased significantly(P<0.05),the effect is better than the western medicine group.5.Comparison of clinical efficacy: after 2 weeks of treatment,the total effective rate of electroacupuncture combined with ginger moxibustion group was 56.6%;the total effective rate of western medicine group was 46.6%,there was no statistically significant difference between the two groups(P>0.05).After 4 weeks of treatment,the total effective rate of electroacupuncture combined with ginger moxibustion group was 86.6%;the total effective rate of western medicine group was 63.3%,and the difference was statistically significant(P<0.05).Conclusion:In summary,1.Electroacupuncture combined with ginger moxibustion and western medicine have certain effects in the treatment of wind-cold bundle phenotype urticaria,and the effect of electroacupuncture combined with ginger-separated moxibustion is better.2.Compared with the western medicine group,short-term electroacupuncture combined with ginger moxibustion has obvious advantages in reducing the weekly frequency of wind-cold chronic urticaria.In the long-term treatment course,it can reduce the number of wind clusters,the frequency,the duration,and improvement of quality of life,improvement of bad mood and other aspects are significantly better than western medicine,the long-term effect of electroacupuncture combined with ginger moxibustion is significantly better than western medicine.Western medicine does not show long-term effect.Electroacupuncture combined with ginger-separated moxibustion has a definite clinical effect in improving wind-cold urticaria.It has the characteristics of high safety,no need for drugs,and resistance to recurrence.It is worthy of clinical promotion. |
