Investigation And Research On The Awareness And Demands Of Elderly With COPD Under Different Elderly Care Models In Urumqi

Objective:To investigate the awareness and demands of COPD of the elderly under different elderly care models in Urumqi;understanding the development of COPD health services in different elderly care models;analyzing the factors affecting the awareness of COPD of the elderly in order to improve the elderly COPD awareness.Methods:From July to October 2019,simple random sampling and convenient sampling methods were used to distribute 1,360 questionnaires in 8 community and community health service centers,6 elderly care institutions,and 5 integrated medical care and elderly care institutions in Urumqi,and finally valid questionnaires 1340 copies.SPSS23.0 was used for data analysis,single factor analysis was performed by x~2test;influencing factors were analyzed by binary logistic regression analysis.Results:(1)In this study,there were 576(43.0%),464(34.6%),and 300(22.4%)elderly people in the home care model,institutional care model,and integrated medical care and elderly care model,respectively.(2)COPD health-related awareness:The overall knowledge awareness rate of COPD for the elderly in the home care model,institutional care model,and medical care combined care model are 18.2%,17.9%,and 27.3%,respectively.The overall knowledge awareness rate of COPD of the elderly in the integrated medical care and elderly care model is higher than that of the home care model and institutional care model(P<0.05),but the overall awareness level of the elderly is not good.(3)Age,children’s financial support,basic medical insurance type,COPD prevalence,and lung function testing are the influencing factors of the overall knowledge awareness rate of COPD in the elderly in the home care model(P<0.05);age,children’s financial support,monthly income,smoking status is an influencing factor of the overall knowledge awareness rate of COPD in the elderly in the institutional care model(P<0.05);age,education level,children’s visit frequency,COPD prevalence,and lung function testing are the influencing factors of the overall knowledge awareness rate of COPD in the elderly in the integrated medical care and elderly care model(P<0.05).(4)COPD health service demand analysis:The top 3health services in the demand rate of the elderly are"high-risk population screening","pulmonary function testing in the national physical examination",and"disease health education",with a demand rate of 72.2%.,64.5%,59.1%.(5)The development and problems of COPD health services in different elderly care models:Some elderly care facilities have carried out smoking cessation education and COPD health education.The three elderly care models are all affected by the lack of professional medical resources,lack of financial security,heavy daily care workload,lack of standardized operation basis,and weak awareness of the hazards of COPD among staff made COPD health service launched.Conclusion:At this stage,the elderly’s COPD health-related awareness level under different elderly care models in Urumqi is low;the elderly’s COPD health service demands are concentrated on high-risk population screening,lung function testing,and disease health education;due to the lack of professional medical staff,lack of financial support,factors such as heavy daily care work hinder the further development of COPD health services.We should give full play to the synergy of medical resources and elderly care services,actively carry out health services such as health education,screening of high-risk groups,and gradually establish COPD prevention and control strategies focusing on health interventions under different elderly care models,in order to achieve the threshold of COPD disease prevention and control shift.
