The Clinical Effect Of "Dragon Tiger Fighting" Acupuncture On Sciatica Secondary To Lumbar Disc Herniation

Objective: To study the effect and feasibility of dragon tiger engagement acupuncture in the treatment of sciatica secondary to lumbar disc herniation.Methods: From October 2018 to November 2019,60 cases were selected from the outpatient and ward of Acupuncture Department of Shijiazhuang Hospital of traditional Chinese medicine and met the inclusion criteria.Randomly divided into two groups,the treatment group(Dragon Tiger belligerent acupuncture group)30 cases,the control group(conventional acupuncture group)30 cases.The main points of the two groups were three lumbar acupuncture points(including two side Shenshu,large intestine Shu and the affected side Weizhong),lumbar Jiaji points(the protrusion of lumbar intervertebral disc and its adjacent upper and lower sides of the intervertebral disc),Huantiao(the affected side)and Ashi points.According to the syndrome differentiation of meridians and collaterals,the points along the bladder meridian of foot Taiyang were selected: Zhibian,Chengshan,Kunlun;and along the gallbladder meridian of foot Shaoyang: Yanglingquan,Xuanzhong,Qiuxu.The main points of the treatment group were treated by the Dragon Tiger belligerent acupuncture technique,the other points were treated by the routine acupuncture technique,and the other points in the control group were treated by the routine acupuncture technique.The two groups were treated with acupuncture once a day,5 times in a row.After each course of treatment,they had a rest for 2 days to enter the next course of treatment,a total of 3 courses of treatment.The clinical effects of the two groups were evaluated by digital pain score(before treatment,after the first treatment,after the first,second and third treatment courses),TCM syndrome score(before treatment,after the third treatment course),and lumbar disease comprehensive score(before treatment,after the third treatment course).SPSS 23.0 was used to analyze the data.Results: The baseline comparison between the treatment group and the control group,including age,gender,course of disease,meridian syndrome differentiation type,digital pain score,TCM syndrome score,lumbar disease comprehensive score,were all P > 0.05,which was comparable between the two groups.(1)Digital pain score(NRS score): there was a significant difference in NRS score between the two groups before and during the treatment(P <0.01),and there was a statistical or significant difference between the two groups during the treatment(P < 0.05 or P < 0.01).There was significant difference between NRS score and baseline difference before and after treatment(P < 0.05).From the end of the second course of treatment,there was no significant difference between the treatment group and the baseline(P > 0.05).(2)TCM syndrome score: after treatment,the scores of main symptom,secondary symptom and concurrent symptom in TCM syndrome score of the two groups were reduced,and there was significant difference between the two groups(P < 0.01);after treatment,there was statistical difference between the two groups(P < 0.05).(3)Comprehensive score of lumbar disease(JOA score): through the comparison of JOA score before and after treatment,there was significant difference in subjective symptoms,clinical signs and daily activities between the two groups(P < 0.01);there was no significant difference in JOA score between the two groups(P > 0.05).(4)Overall effect: the total effective rate was 93.33% in the treatment group and 86.20% in the control group.The difference between the two groups was statistically significant(P < 0.05).The clinical effect of thetreatment group is better than that of the control group.(5)There were no serious and adverse events in the whole course of treatment,59 patients’ basic vital signs had no abnormal changes,suggesting that the safety of this study was good.Conclusion:(1)Dragon Tiger belligerent acupuncture and conventional acupuncture can improve the degree of pain,TCM syndrome and lumbar function of patients with sciatica secondary to lumbar disc herniation.(2)Longhu belligerent acupuncture has the same effect as conventional acupuncture in improving the lumbar function of patients with sciatica secondary to lumbar disc herniation.It is better than conventional acupuncture in improving the degree of pain and TCM syndromes,and it can quickly relieve the degree of pain in the early stage of treatment,with stronger timeliness.(3)Longhu belligerent acupuncture is superior to conventional acupuncture in the treatment of sciatica secondary to lumbar disc herniation,and has good safety.
