Objective: This study independently developed the quality of life scale(EBQo L)based on environmental factors. The integration system of the EBQo L scale was constructed by discrete selection experiment(DCE)to obtain the health utility value of the population based on environmental preference in China. To provide the optimal choice and basis for decision makers in different intervention programs. At the same time, this study used three kinds of scales to explore the comparison of the results of life quality measurement scores of different populations,in order to provide a reference for the health improvement of Chinese population and the selection of life quality measurement tools. Methods: According to the geographical location and feasibility of the study, the general population, patients with diabetes, hypertension and chronic hepatitis were selected in shenyang, shijiazhuang, nanjing, guiyang and sichuan. The EQ-5D,SF-6D and EBQo L scale based on environmental factors were used for questionnaire survey. The conditional logit model was used to construct the integral system of EBQo L scale through DCE.Descriptive analysis was conducted on the quality of life of the general population, diabetes, hypertension and chronic hepatitis in China.Pearson correlation coefficient,ICC correlation coefficient and Tobit regression were used for analysis. Results: A total of 1929 subjects were included in the survey, including 1207 patients,241 patients,241 patients and 240 patients with diabetes, hypertension and chronic hepatitis. There were 956 males(49.56%)and 973 females(50.44%).The youngest age of the respondents was 18,the oldest was 91,and the average age was(45.45±17.30).The majority of respondents were married(1433 cases,74.33%).In terms of scale scores, the scores of EQ-5D-5L,SF-6D and EBQOL were(0.955±0.069),(0.810±0.147)and(0.624±0.188),respectively.The scores of diabetic patients were(0.872±0.142),(0.688±0.204)and(0.656±0.211)respectively.The scores of hypertension patients were(0.880±0.170),(0.695±0.213)and(0.640±0.211),respectively.The scores of chronic hepatitis patients were(0.882±0.147),(0.699±0.207)and(0.613±0.199),respectively.The total scores of the three scales measured by range standardization method are correlated to a certain extent.(r=0.153~ r=0.677,P < 0.01).The results of single factor and Tobit regression analysis showed that alcohol consumption, serious illness and happiness had an impact on scores of EQ-5D-5L,SF-6D and EBQo L scales(P < 0.001).Conclusion: Overall, the quality of life of the general population and the slow patients(diabetes, hypertension, hepatitis)surveyed in this study was better. Most of the respondents focused on physical pain and mental state. For the quality of life based on the environment, air quality, living environment and other indispensable living conditions for human beings are the most worrying problems of most respondents. At the same time, the quality of life of the respondents is closely related to the physical health status, mental health status, personal living environment and conditions, natural environment, social environment, etc. The improvement of their health is not only limited to the traditional micro-personal physical and mental health, but also needs to pay attention to the macro-external environment of the population, and comprehensively improve the quality of life of the population.and for indirect acquisition of health utility values for people with multiple diseases, when the health utility values of people with multiple diseases can not be directly obtained,it is an effective choice to use the health utility values of a single disease state for estimation. To sum up,in the actual operation of measuring the quality of life of the population, because of the uncertainty of the individual in self-evaluation,the complexity of the influencing factors of health, the influencing factors of measuring the quality of life score of each scale are still different. Therefore,in the future similar research can be based on the object of investigation and research program to choose the appropriate quality of life measurement tools. |
