Analysis Of The Characteristics And Treatment Of Intertrochanteric Fracture

Objective:To analyze the characteristics of intertrochanteric fracture in Shandong hospital of traditional Chinese medicine,and to observe the therapeutic effect of traditional Chinese and Western medicine,so as to provide a plan for further improving the diagnosis and treatment of intertrochanteric fracture.Methods:Through the medical record system of the First Affiliated Hospital of Shandong University of traditional Chinese medicine,the clinical data of patients hospitalized with intertrochanteric fractures July 2014 and December 2018 were collected.To analyze the relationship between the age,sex,injury cause,season,occupation,smoking history,drinking history and fracture,to explore the effect of treatment and complications on fracture curative effect,and to summarize and analyze the characteristics of fracture.46 patients with intertrochanteric fracture admitted to the Department of micro orthopaedics,the First Affiliated Hospital of Shandong University of traditional Chinese medicine from December 2017 to December 2018 were randomly divided into observation group(23 cases)and control group(23 cases)according to the order of admission.Both groups were treated with internal fixation of PFNA.After operation,they were given conventional drugs such as anti infection,detumescence and anticoagulation.After operation,they were sterilized once every two days.On the basis of routine treatment,the observation group used the hospital preparation Ma Huang tincture(Strychnos nux vomica,Coptis,etc.)wet compress in the wound during aseptic dressing change,while the control group only used routine basic treatment and aseptic dressing change,and used the blank control.The leukocyte count,ESR,C-reactive protein and visual analogue score of the patients in the two groups were observed after operation.Results:1306 cases of intertrochanteric fracture were collected by the electronic medical record system of the First Affiliated Hospital of Shandong University of traditional Chinese medicine.The results showed that most of the patients were farmers,and the main pathogenic factor was falling injury.The number of female patients was significantly more than that of male patients,the age was concentrated between 60 and 80 years old,and the number of patients in winter and spring was significantly more than that in summer and autumn.Among them,1057 patients were treated by operation,most of them were treated by closed reduction and internal fixation under epidural anesthesia,with an average preoperative preparation time of 6.76 days;249 patients were treated conservatively.Hypertension,coronary heart disease and diabetes are the three major complications of intertrochanteric fracture.40 cases of effective intertrochanteric fracture were treated with PFNA closed reduction and internal fixation combined with external application of Mahuang tincture.The results showed that the difference of hospitalization days,leukocyte count,ESR,C-reactive protein and VAS score between the two groups was statistically significant(P<0.05),which indicated that Mahuang tincture had a definite effect on reducing inflammation and pain after intertrochanteric fracture.Conclusion:Intertrochanteric fracture of the femur is a common and frequently occurring disease in the elderly.Internal diseases are the main factors that affect the treatment effect.The treatment of intertrochanteric fracture of the femur with the combination of traditional Chinese and Western medicine can achieve good results,which is worth popularizing.
