| Objective:This clinical study compares the effect of Guan Jie Tong Li Decoction plus Melilotus Extract Tablet combined with Melilotus Extract Tablet alone on pain swelling and joint function recovery after supination external rotation ankle fracture Ⅳ° surgery,evaluates the clinical efficacy of Guan Jie Tong Li Decoction,and provides a more effective treatment scheme for quick and better recovery of ankle function after surgery.Methods:In this study,38 patients with supinator external rotation ankle fracture Ⅳ°admitted to the Department of Trauma and Orthopedics of Shandong Bone Trauma Hospital from April 2018 to April 2019 were randomly divided into two groups: group Guan Jie Tong Li Decoction combined with group Melilotus Extract Tablet(hereinafter referred to as the treatment group)and group Melilotus Extract Tablet alone(hereinafter referred to as the control group)according to the inclusion and exclusion criteria of this study.Two cases dropped out during the study,36 cases were final effective samples,and18 cases were treated group and control group,respectively.The two groups were treated with routine edema reduction before surgery,followed by open reduction and internal fixation,and ankle brace fixation after surgery and routine functional exercises under the guidance of doctors.The treatment group began to take Guan Jie Tong Li Decoction and Melilotus Extract Tablet on the first day after operation,and the observation group took Melilotus Extract Tablet on the first day after operation.The ankle swelling score and visual analogue scale were observed and compared between two groups of randomized trial patients at 3 days,1 week,2 weeks and 1 month after treatment;Assess the recovery of joint activity in the two groups at 3 days,1 week and 2 weeks after treatment;observe the difference of AOFAS ankle and hind foot scores in the two groups at 3 days,1 month and 3 months after treatment;evaluate the change level of HS-CRP,an inflammatory index in the two groups at 1 day,3 days and 1 week after treatment.The clinical data of 36 patients were statistically analyzed using statistical software SPSS23.0,and the changes of related indicators in the two groups were observed and compared to evaluate the clinical efficacy of the two groups.Results:1.Postoperative pain and ankle swelling The swelling and pain of the two groups were still obvious 3 days after treatment,and there was no significant difference between the two groups(P>0.05);the pain of the treatment group was significantly lower than that before treatment at 1 week,2 weeks and 1 month after treatment(P≤0.05),and the improvement of the treatment group was better than that of the control group(P≤0.05);the swelling relief of the treatment group was lower than that before treatment at 2 weeks and 1 month after treatment(P≤0.05),and the detumescence effect of the treatment group was better than that of the control group(P≤0.05);2.Joint range of motion recovery in the treatment group 3 days,1 week,2 weeks after treatment,the improvement of joint range of motion was significantly higher than that before treatment(P≤0.05),and the improvement of the treatment group was better than that of the control group(P≤0.05);3.Joint function recovery There was no significant improvement in joint function and no significant difference in AOFAS score between the two groups 3 days after treatment(P> 0.05);AOFAS score in the treatment group was significantly higher than that before treatment at1 month and 3 months after treatment(P≤0.05),and the effect of joint function recovery in the treatment group was better than that in the control group(P≤0.05);4.Inflammatory indicators HS-CRP value was higher and no significant difference was found between the two groups at 1 day after treatment(P>0.05);the inflammatory index of the treatment group was significantly lower than that of the control group at 3 days and 1 week after treatment(P≤0.05),and the improvement effect of the treatment group was better than that of the control group(P≤0.05).Conclusion:The Guan Jie Tong Li Decoction can effectively alleviate the pain and swelling after the operation of supination external rotation ankle fracture Ⅳ°,improve the range of motion of the joint,inhibit the inflammatory reaction,and play a good role in promoting the recovery of ankle function in the later stage.The clinical effect of combination of Guan Jie Tong Li Decoction and Melilotus Extract Tablet is better and significant.Both of them play a synergistic role to enhance the clinical efficacy.The therapeutic effect is significantly better than that of Melilotus Extract Tablet alone,which is conducive to early functional exercise and recovery of ankle function after surgery.The treatment method has definite curative effect,high safety,economical and convenient operation,and has significant clinical value,which is suitable for wide popularization and application. |
