Discussion On The Clinical Experience And Characteristics Of Using Wuling San By Chief Physician Yuetao Gu Based On Medical Record Arrangement

Objective:Wuling San,documented in Zhongjing Zhang’s<Shanghanlun>,is a famous prescription for treatment of fluid and Qi disorders.Wulingsan imparts its therapeutic activity through warming Yang,promoting Qi,and resorting fluid balance.In history,Wuling San has been used by physicians to treat in the field of internal medicine,gynecology as well as pediatric.Professor Yuetao Gu is experience in utilizing Wuling San to treat chronic kidney diseases and has achieved remarkable clinical outcomes.It is of great value to research the mechanisms of Wuling San and professor Yuetao Gu’s therapeutic approach.This paper,based on evidence from literature,clinical case reports,data analysis and clinical experience,will systematically discuss the clinical applications of Wuli ng San,the pathogenisis of ’Wuling San syndrome’,the relationship between Wuling San and Ziyi Peng’s circle theory,and analysis of professor Yuetao Gu’s clinical experience.Method:This paper covers a range of analysis of the clinical application of Wuling San.The materials used for analysis include clinical cases<Shanghanlun>,literatures related to<Shanghanlun>,physicians’ clinical reports of Wuling San,contemporary research of Wuling San,pharmacological research,and circle theory.Professor Yuetao Gu’s clinical experience related to Wuling San is summarized through a prism of analysis of clinical data available on traditional Chinese medicine platform as well as clinical experience.Part 1 Literature reviewA summary of Wuling San’s clinical application and pharmacological mechanisms based on data from the clinical experience of Zhongjing Zhang and other physicians.Part 2 Analysis of chief physician Yuetao Gu’s clinical experience of Wuling San.By using the auxiliary platform system of traditional Chinese medicine inheritance,the data of the medical records are analyzed,and the clinical experience and drug characteristics of Professor Gu Yuetao’s application of Wuling San are systematically summarized in close combination with clinical practice.Part 3 DiscussionBring the model of circular movement into Professor Gu Yuetao’s experience of using Wuling San to explain his academic thoughts from different anglesResults and conclusions:First.The author found that Wuling san is effective in treating’taiyangxushui’ through analysis of literature and clinical cases.Not only did Wuling san improve fluid disturbances in taiyang bladder,but it is also achieve therapeutic effects on fluid disturbances in many parts of the bladder meridian of the foot sun and even on fluid disturbance in any parts of the body.As reveled in experiments and clinical findings,Wuling San is effective in improving nephrotic syndrome,chronic heart failure,diseases with edema as main manifestation such as postoperative edema,and diseases or procedures that induce dehydration such as hemodialysis.This indicates that Wuling San is not simply a diuresis;it also imparts regulatory effects on urine in a bidirectional manner,meaning that Wuling San has antidiuretic effects under dehydration state.Thus,two important conclusions can be drawn:1.All patients suffer from fluid imbalance can be treated with Wuling San or adjustments of Wuling San.2.When patients suffer from wither edema or fluid insufficiency,the use of Wuling San should be considered as it may help to restore fluid balance through bidirectional regulation of urine.Two.According to Professor Gu Yuetao’ s experience in using Wuling San,the original or modified Wuling San is the best treatment for all mild and moderate phlegm dampness and water drinking syndromes;in patients with chronic kidney disease,if the pathogenesis is "deficiency of spleen and kidney,unsteadiness of convergence,stagnation of water and blood stasis",Wuling San can be used to remove Guizhi plus tonic and astringent drugs,promoting blood circulation and benefiting water,so as to teach Gu Yuetao The basic prescriptions commonly used in teaching are:Polyporus,Pori a cocos,Alisma oriental is,Atractylodes macrocephala,sayuanzi,Rosa laevigata,yam,agrimony,Rubia,cuttlebone.The basic formula is only applicable to some patients with chronic kidney disease who conform to the pathogenesis.In clinical practice,we should pay attention to the main syndrome,syndrome differentiation and treatment,and then add and subtract changes according to different stages of the disease.Three.From the perspective of "round movement",the pathogenesis of Wuling San is that the water channel is blocked by evil,which makes the water evil stop gathering all over the body,and then leads to the imbalance of round movement of water and gas.The evil of water and Qi is Yin,which can not only affect the burning effect of phase fire,but also affect the transportation of spleen and stomach,hinder the meridians and affect the release of wood Qi.Finally,a stalemate situation of water and gas stagnation,unfavorable transport and chemical,and unsmooth drainage will be formed.To a certain extent,the use of circular movement model can supplement and explain the pathogenesis of Wuling San,which is helpful to open up clinical thinking.
