Object:To systematically evaluate the efficacy and safety of external application of Chinese medicine in the treatment of primary liver cancer pain,which guides clinical work of treatment of primary liver cancer pain.Method:Searching for clinical Randomized controlled trial of external application of Chinese medicine for treatment of primary liver cancer pain in Cnki,CBM,Wanfang,VIP,Pubmed,Cochrane Library and Embase,screening of eligible literature according to the inclusion and exclusion criteria,the relevant data of pain relief rate,pain score,improvement rate of KPS and incidence of adverse reactions were collected and analyzed with revman 5.3 software.Result:A total of 1389 articles were retrieved from Chinese and English databases,and 24 Chinese studies were selected according to inclusion and exclusion criteria.A total of 1754 patients with primary liver cancer pain were included,with a total of 899 cases in the experimental group and 855 cases in the control group.The result showed that the pain relief rate of external application of Chinese medicine in the treatment of primary liver cancer pain was higher than that of three-step analgesic therapy,RR=1.30,95%CI=(1.21,1.40),Z=7.16,P<0.00001;the pain score was lower than that of three-step analgesic therapy,SMD=-2.48,95%CI=(-3.40,-1.57),Z=5.30,P<0.00001;the improvement rate of KPS was better than that of three-step analgesic therapy,RR=1.32,95%CI=(1.09,1.60),Z=2,87,P=0.004;the incidence of nausea and vomiting was lower than that of three-step analgesic therapy,RR=0.33,95%CI=(0.15,0.71),Z=2.82,P=0.005;the incidence of constipation was lower than that of three-step analgesic therapy,RR=0.28,95%CI=(0.13,0.62),Z=3.15,P=0.003;the incidence of hypersomnia was not significantly different from that of the control group,RR=0.43,95%CI=(0.07,2.72),Z=0.90,P=0.37.Conclusion:External application of Chinese medicine in the treatment of primary liver cancer pain has a definite clinical effect,can improve the clinical symptoms of patients,reduce adverse reactions.The Meta-analysis of this study showed that the external application of Chinese medicine in the treatment of primary liver cancer pain was superior to the three-step analgesia therapy in sides of pain relief rate,pain score and so on,and less adverse reactions,however,due to the lack of high-quality studies on external application of Chinese medicine for the treatment of primary liver cancer pain at home and abroad,the quality of the literature included is not high,which may have some impact on the results of this study,and more high quality clinical Randomized controlled trial studies support are needed.
