The Nineteenth National Congress defines the main contradiction of society as the contradiction between people’s need for a better life and "unbalanced and inadequate".From the perspective of medical resources supply and demand,it can be understood as the contradiction between "limited and high-quality medical resources demand" and "uneven distribution,population,technology,and medical resources supply that does not match the degree of urbanization".The function of medical consortium is to realize the vertical integration of medical resources,improve the overall efficiency of medical resources,and solve various contradictions caused by the uneven supply and demand of medical resources.Since 2010,Shanghai has been committed to the construction of regional medical consortium.After years of hard practice,the operation of Shanghai regional medical consortium has played a substantial role in optimizing the allocation of medical resources,achieving orderly medical treatment,strengthening the division of labor and cooperation among medical institutions,and solving the contradiction between the supply and demand of medical resources.However,due to the particularity of Shanghai’s urban area and the constraints of population aging,the distribution of medical resources,uneven distribution,the defects of top-level system design and the influence of potential factors,the mode and implementation of Shanghai Regional Medical Consortium are difficult to meet the needs of many aspects,long-term and new forms of development.It is necessary to explore the optimal path of the mode of Shanghai Regional Medical Consortium in the light of the actual situation in order to meet the current needs.On the premise of the requirement,it can meet the needs of more forms in the future,so as to give full play to the role of the Medical Association in optimizing the allocation of medical resources,assist the implementation of the graded diagnosis and treatment system and strengthen the construction of grass-roots units.The operation and construction of the Medical Consortium is a huge and complex systematic project.We should strengthen the integrity,systematicness,complexity and synergy in the process of regional medical consortium construction,give full play to the ability of the Medical Consortium model to solve the problems related to the allocation of medical resources,grasp the essential connotation and generating logic of the operation of the Medical Consortium,crack the difficulties of the operation mechanism of the Medical Consortium,and give full play to its medical treatment.The rational allocation of resources is of great practical significance at this stage,and it is also an urgent task for the construction of the Medical Union at present.From the perspective of demand for medical resources and the contradiction between supply and demand,this paper expounds the concept,development and implementation of the medical association,its position and role in the reform of medical system,and its characteristics based on the government mechanism.It also focuses on the background,current situation,characteristics and problems of the construction of the Medical Association in Shanghai.With the help of the domestic and foreign academic circles,it makes a thorough study of the medical association.In the second chapter,the theoretical and logical connotations of the model of medical consortium are elaborated and explained by using system theory,synergy theory and Pareto optimality of resources,and the definitions of medical consortium and regional medical consortium are distinguished.This paper summarizes and analyses the development of regional medical consortia in China at present,and summarizes the operating mechanism and characteristics of the three medical consortia modes according to the operating logic of the medical consortium modes in China,and analyses their generating logic,influencing factors and operating characteristics.Chapter III takes the comparative study of the British and American medical consortia modes as the starting point,and analyses the background of the British and American medical consortia modes.Characteristic,implementation effect and reference to the development of China’s medical association;Chapter IV,starting from the current situation of the overall allocation of medical resources in Shanghai,analyses the significance of building the Regional Medical Association Model under the current situation of Shanghai’s medical resources;Through the analysis of the characteristics,implementation effect and influencing factors of the Ruijin-Luwan,Changning and Chongming-Xinhua medical Association model,it analyses Shanghai’s implementation effect and influencing factors.Based on the overall implementation background,implementation degree and specific impact indicators on the allocation of medical resources,the fifth chapter constructs the evaluation index system of influencing factors of the Shanghai Regional Medical Association,and evaluates the overall effect of the implementation of the Shanghai Regional Medical Association from 2011 to 2016,the relationship between influencing factors and the development trend of influencing factors in the future.Quantitative analysis was carried out to objectively analyze the overall effect of the implementation of the Shanghai Regional Medical Association,the degree of optimal allocation of medical resources,advantages and disadvantages.Based on the analysis conclusion and the current external collaboration,efficiency priority,full collaboration and innovation and development of the model.
