[OBJECTIVE ]To observe the clinical efficacy of Xiaoyusan plaster combined with acupotomy on the treatment of knee osteoarthritis(KOA).and to observe the effect of Xiaoyusan plaster on articular cartilage of rabbits knee osteoarthritis(KOA),explore the theoretical connotation of traditional Chinese medicine new methods.[METHODS]Clinical study part:a total of 150 KOA patients were enrolled in a randomized controlled clinical trial.They were randomly divided into group A(Xiaoyusan plaster combined with acupotomy group),group B(Xiaoyusan plaster group),and group C(Votalin group).two weeks were treated continuously.evaluating the efficacy and detect the changes of inflammatory factors before and after treatment.Fundamental study part:42 6-month-old healthy New Zealand white rabbits were randomly divided into group A(normal group),group B(model group),group C(Xiaoyusan plaster group),group D(Xiaoyusan New Formula group).animals KOA model were made and verified,after interventing,articular cartilage samples were evaluated and taken for testing.[RESULTS]Clinical study part:(1)The total effective rate in group A was significantly higher than that in group B and C(P < 0.05).(2)After treatment,the WOMAC scores in the three groups decreased(P < 0.05),and the scores in group A were lower than those in group B and C(P <0.05).(3)After treatment,the VAS scores in the three groups decreased(P< 0.05),and the scores in group A were lower than those in group B and C(P < 0.05).(4)The expression levels of IL-1βand TNF-αin the three groups decreased after treatment(P < 0.05),and IL-1βin group A was lower than that in group B(P < 0.05).Fundamental study part:(1)Gross observation of Outerbridge score showed that the number of high-grade animals in group C and group D was lower than that in group B(P < 0.05),and that in group D was lower than that in group C(P < 0.05).(2)HE staining Mankin’s score of articular cartilage:the score in group C and D was lower than that in group B(P <0.05),and that in group D was lower than that in group C(P <0.05).(3)Comparing the positive rate of MMP-13 expression cells:compared with group B,the number of MMP-13 positive cells in group C and D decreased,that in group D less than that in group C(P < 0.05).[CONCLUSION]Xiaoyusan plaster combined with Acupotomy is effective and safe on the treatment of knee osteoarthritis,which is worthy of clinical application.Xiaoyusan plaster and Xiaoyusan New Formula can inhibit articular cartilage degeneration in KOA rabbits,the level of MMP-13 decreased,which may be one of the mechanisms of the treatment.
