| ObjectiveThis study aims to observe the clinical efficacy and safety of the filiform-fire needle in treating adolescent mild and moderate acne,and explore its mechanism and advantage primarily,in order to provide faster and more effective method for clinical treatment of acne and further lay a foundation for fire needle treatment.MethodsSixty-two adolescent patients with mild and moderate acne vulgaris were randomly divided into treatment group and control group,each with 31 cases.The treatment group was treated dialectically with mile-fire acupuncture therapy.7-8 skin lesions of face,chest and back were selected to prick,to remove blackheads or pustules secretions,and then prick acupoints with syndrome differentiation once every 1 day and once every 2 weeks for a course of treatment(Feishu and Dazhui were selected for wind-heat syndrome;Weishu and Dachangshu were selected for damp-heat syndrome;Ganshu,Shenshu and Geshu were selected for disharmony of thoroughfare).In the control group,Benzoyl Peroxide Gel was applied to local skin lesions once during the day,and Tretinoin Cream was applied to skin lesions once before bed.After two courses of treatment and 1 month fllowed-up,the efficacy and recurrence rate of the two treatment methods was evaluated and analyzed in both groups,and draw a conclusion.ResultsAfter 4 weeks of treatment,a total of 62 patients were collected,of which 30 patients were in the treatment group,including 1 patient lost,and 29 patients were in the control group,including 2 patients lost.1.Comparison of baseline data:Before treatment,the gender,severity of acne and syndromes of traditional Chinese medicine(TCM)of patients in the two groups were tested by chi-square test(P>0.05),indicating that the difference of the gender,severity of acne and TCM syndromes in the two groups have no statistical significance.The mann-Whitney U rank sum test of the age of the patients in the two groups before treatment showed that the age difference between the two groups was not statistically significant(P>0.05).Before treatment,the distribution,quantity,degree,color,swelling and pain,and the total integral of acne lesions in the two groups were tested by t test when the normal distribution and homogeneity of variance were met,while those who did not met the normal distribution were tested by mann-whitney U rank sum test.And the tests(P>0.05)showed that there was no significant difference between the two groups in the distribution,quantity,degree,color,swelling and pain,and the total score of acne lesions.In summary,there was no significant difference in baseline data between the two groups,which was comparable.2.Comparison of efficacy:(1)After 4 weeks of treatment,there was a significant difference in the distribution and the total integral of acne lesions between the two groups(P<0.05)after the mann-whitney U rank sum test,while there was no significant difference in the quantity,degree,color and swelling and pain of skin lesions between the two groups(P>0.05).(2)After 4 weeks of treatment,8 cases in the treatment group were cured,16 cases were conspicuously effective,3 cases were effective,3 cases were ineffective,and the total effective rate was 90.0%.While in the control group,there were 5 cases recovered,9 cases were conspicuously effective,6 cases were effective,9 cases were ineffective,and the total effective rate was 69.0%.There was a significant difference in the total effective rate of the two groups tested by Chi-square test(P<0.05).(3)After 4 weeks of treatment,there was no significant difference in the efficacy of three TCM syndromes of wind-heat syndrome,damp-heat syndrome and disharmony of thoroughfare,tested by Chi-square test(P>0.05).(4)After 1 month of follow-up,all 47 patients with effective treatment were followed up.Compared with those before treatment,1 patient in the treatment group had recurrence,and 4 patients in the control group had recurrence.The recurrence rate of the two groups was tested by chi-square test,with no significant difference(P>0.05).(5)During the treatment,No serious adverse events occurred in the two groups,indicating that the two therapies are safe and effective.Conclusion1.Both mile-fire acupuncture and western ointment group had good effect on adolescent acne vulgaris,but the curative effect of mile-fire acupuncture group was better than that of western ointment group2.Compared with the western ointment therapy,the mile-fire acupuncture therapy is more effective in improving the overall skin lesions in adolescent acne vulgaris patients,especially in reducing the distribution of skin lesions.3.The curative effect of mile-fire acupuncture therapy is similar to that of western ointment therapy in improving the quantity,degree,color,swelling and pain of skin lesions in adolescent acne vulgaris patients.4.The efficacy of needle therapy is comparable to that of western ointment in preventing acne recurrence.5.There has a curative effect of mile-fire acupuncture therapy in dialectical treatment of acne of wind-heat syndrome,damp-heat syndrome and disharmony of thoroughfare.The fire needle treatment has slight adverse reaction,safe and easy operation,which is worthy of clinical promotion and application. |
