Objective:To assess the cerebral blood flow(CBF)basing on three-dimension arterial spin labeling(3D-ASL)in predicting neurological recovery in patients with acute ischemic stroke(AIS).Methods:By reviewing the cases of 70 patients with acute ischemic stroke by the means of the 3D-ASL image and NIHSS(National Institutes of Health Stroke Scale Score),ROI is manually selected on DWI.After that,the CBF at the infarct side and the contralateral side were measured by overlaying the DWI image on the ASL perfusion image.Hence,r CBF was formed by their ratio.Lastly,this study took paired t tests for the values of CBF,r CBF and NIHSS before and after treatment respectively.The correlation of CBF changes and NIHSS scores before and after treatment was made by spearman rank correlation test.Results:There was significant difference in the changes of CBF,r CBF,and NIHSS before and after treatment(P<0.05).The CBF of the infarct area turned higher in 63 patients after treatment,and 62 of them whose NIHSS decreased while 1 increased.The CBF of the infarct area turned lower in 7 patients after treatment,among which 2 patients’ NIHSS showed no change and 5 patients’ increased.There was a significant negative correlation between CBF changes in infarct area and NIHSS score before and after treatment(r=-0.39,P<0.05).Conclusion:1.the value of cerebral blood flow in abnormal perfusion area of cerebral tissue measured by 3D-ASL technique can be used for prognosis evaluation,and the recovery of cerebral blood flow can help the recovery of nerve function in patients with cerebral infarction.2.as a MRI perfusion technique without injection contrast enhancement,three-dimensional arterial spin labeling simplifies the acquisition of CBF.the safety,non-invasive and rapid advantages make it more suitable as a review sequence after treatment in stroke population.it is an important imaging method that can be applied to the evaluation of functional recovery in acute stroke.
