ObjectiveFunctional dyspepsia,as a common disease in modern society,seriously affects people’s quality of life.The treatment of functional dyspepsia in Chinese and western medicine is focused on symptomatic treatment.However,western medicine has obvious side effects and insufficient evidence of long-term efficacy.This study first of all,from the perspective of pathogenesis of traditional Chinese medicine and western medicine disease for down-regulated method in the treatment of functional dyspepsia theoretical basis,after on the one hand,the quantitative analysis method of down-regulated by system evaluation means for the treatment of functional dyspepsia clinical efficacy,for down-regulated method in the treatment of functional dyspepsia clinical evidence;On the one hand,the calculation of electricity physiology and molecular biology methods analysis method for the treatment of functional dyspepsia targets of down-regulated,from down-regulated method for treatment of functional dyspepsia for laboratory evidence.enlivenMethods1.Literature studySubjects:published in China,To study the randomized controlled literature on the clinical efficacy of invigorating spleen and regulating qi in the treatment of functional dyspepsia.Literature sources:Chinese Biomedical Literature Database(CBM),Chinese Biomedical Literature Database(CNKI),Weipu Information Network(VIPN),Wanfang data knowledge Service platform and PubMed database.At the same time,the manual retrieval of relevant journals’ professional data(starting years of each database from 1999 to 2018)was included in criteria 1:clinical randomized controlled trial research.,but the experiment can do double blind,single blind and non blind treatment.The main intervention measure of the clinical treatment is to strengthen the spleen and regulate qi for the treatment of functional dyspepsia.The clinical treatment of the control group was to use western medicine or placebo alone in the treatment of functional dyspepsia.The evaluation criteria for the clinical efficacy of the three groups were clear and accurate.The data can be calculated correctly.Excluding the standard of 1:1 animal experimental study intervention measures combined with other traditional Chinese medicine prescriptions,acupuncture,auricular points,and other treatments,or combined with western medicine treatment,although it is a randomized controlled trial,However,the evaluation index of therapeutic effect was not standardized or detailed in self-controlled trial.Patients with obvious errors or defects in the clinical trial design without statistical basic data were published.Then the bias risk of the study was evaluated by two evaluators according to the Cochrane handbook for RCT bias risk assessment tools.2.experimental study48 SD male rats with SPF grade were randomly divided into blank group and model group according to the random digital table.Among them,8 rats in blank group were fed normally in group A.No intervention was made.The rat model of spleen deficiency syndrome with FD visceral hypersensitivity was established by iodide acetamide,small platform method and fasting every other day in the model group.Forty successful rats were randomly divided into 5 groups:model group,low dose group,middle dose group and high dose group.Each dose group of Jianpi Liqi granule was treated with Jianpi Li Li respectively.The rats were given mosapride for 14 days.The symptoms and body weight of each group were observed,and the content and expression of IL-6 IL-10 in serum of rats were detected by ELISA method.Result1.Literature studyFinally,15 articles on randomized controlled clinical trial of invigorating spleen and regulating qi for treatment of functional dyspepsia were included.The intervention measures of the study object and the selection of outcome indexes could represent the patients with functional dyspepsia.There is a certain risk of bias and low heterogeneity in the literature judging from four aspects:random allocation method,allocation scheme hiding,blind method application and outcome loss.The conclusion is more scientific and objective.The total effective rate of strengthening spleen and regulating qi in treating FD was better than that of control group,but at present the data could not support the method of invigorating spleen and regulating qi to promote gastric emptying rate of FD and the long-term curative effect was higher than that of control group.2.experimental study(1)the rats showed decreased activity,weak limbs,irritable irritability,withered hair,thin dry defecation,weight loss,and significant reduction in the number of cases,standing and modification of the rats after the establishment of the model.The decrease of activity indicated that the FD model of spleen deficiency and qi stagnation was successfully duplicated.(2)compared with the blank group,the gastric emptying rate in the model group was lower than that in the blank group,and the difference was statistically significant(P<0.05).(3)compared with the blank group,model group rats serum Ghrelin,MTL content decreased,VIP content increased,CCK8,P<0.05;compared with the model group,western medicine group and Jianpiliqi granule in the high dose group of different degree improve the Ghrelin in rat serum,content of MTL,P<0.05 the Western Medicine group;and Jianpiliqi granule in different degree,the high dose group decreased in serum of rats with low content of CCK8,VIP,P<0.05.Conclusion1.Literature studyCompared with the control group and before treatment,spleen qi method in the treatment of functional dyspepsia outcomes improved with statistical significance.But the method of literature quality;intervention regimens;more subjective outcome indicators,objective evidence is less,the lack of long-term follow-up data.To confirm the clinical effectiveness of the treatment of invigorating spleen and regulating Qi function dyspepsia,still need to carry out more high quality multi center,large sample randomized controlled trials.2.experimental studyThe model of FD rats with spleen deficiency and qi stagnation could be successfully established by iodide acetamide+station small platform method and the method of fasting every other day,and the gastric emptying rate and the content of ghrelin motilin MTL in serum of rats could be increased after intervention by Jianpi Liqi granule.The results showed that Jianpi Liqi granule could regulate the dynamic balance between brain and intestine by correcting abnormal levels of brain intestinal peptide,and then improve gastrointestinal motility effectively.
