objective:Explore the prevalence and related risk factors of rheumatoid arthritis combined with depression in Xinjiang.Methods:A retrospective analysis of the name,gender,age,height,weight,ethnicity,education level,marital status,occupation,etc.of RA patients in our department from January 2020 to December 2020.The number of hospitalizations in a year and the treatment expenditure of each hospitalization are obtained from the hospitalization system of our department.HADS to assess depression,and the SF-36 to assess the quality of life,The PSQI assesses sleep quality,the VAS is used for pain assessment,and the DAS-28 assesses the patient’s disease activity.According to the results of the HADS assessment,the patients were divided into two groups with RA combined with depression and without depression.The general information,sleep quality,medical expenditure and quality of life between the two groups were compared,and the correlation and risk factors were analyzed.If the data conform to the normal distribution,the measurement data are all expressed by the mean±standard deviation(x±s),and the t test is used for statistics.Non-normally distributed data is represented by M(P25,P75),and the comparison of continuous variables between the two groups uses the rank sum test.Pearson correlation test is used for continuous variables,and Spearman’s correlation test is used for rank variables or categorical variables.Binary logistic regression analysis of risk factors for RA combined with depression.Use SPSS 26.0 software to complete data statistical analysis.Results:The prevalence of RA combined with depression is 31.5%(100/317),There were statistically significant differences in the scores of DAS28-ESR,DAS28-CRP,PSQI,VAS and SF-36 between the RA with depression and without depression(P<0.05),There were statistically significant differences in total medical expenditures,examination costs,and treatment costs between the two groups of RA with depression and without depression(P<0.05),while the differences in drug expenditures were not statistically significant(Z=-1.153,P>0.05).Correlation analysis shows:DAS28-ESR(OR=0.335 P<0.001),DAS28-CRP(OR=0.335 P<0.001),VAS(OR=0.408P<0.001),PSQI score(OR=0.308 P<0.001)have a weak positive correlation with depression,Physical function(OR=-0.316 P<0.001),physical pain(OR=-0.388 P<0.001),vitality(OR=-0.447 P<0.001),social function(OR=-0.322 P<0.001),emotional role(OR=-0.341 P<0.001),mental health(OR=-0.453 P<0.001)are negatively correlated with depression,There was no correlation between physical role(OR=-0.251 P<0.001)and general health status(OR=-0.296 P<0.001)with depression.The results of multivariate regression showed that VAS(OR=1.028,P<0.001)and PSQI total score(OR=1.145,P<0.001)were independent risk factors for RA combined with depression.Conclusion:The prevalence rate of RA with depression is 31.5%.Compared with patients without depression,patients with depression have higher disease activity,sleep quality and quality of life are poor than patients without depression.The medical expenditure of patients with RA with depression is higher than that of patients without depression;Pain and sleep disturbance are independent risk factors for depression in RA patients.
