| Research Objective:This study is intended to carry out a questionnaire survey among 9 community health service institutions in an administrative area of Xi ’an.To understand the pre-hospital first aid capabilities of community health service institutions in our city,and the medical technicians’ knowledge of community pre-hospital first aid and their work competence.Also to know the demand and satisfaction of community residents for the community’s prehospital emergency work,etc.To propose a feasible model for community health service agencies and participate in pre-hospital emergency work.To construct an appropriate urban pre-hospital emergency system,and put forward relevant policy recommendations for the construction of the system.Research content and methods:1.By consulting literature and related materials,we can learn about the pre-hospital first aid system at home and abroad.The model of community health service institutions participating in pre-hospital first aid is studied to provide ideas and directions for this investigation.2.This research is a descriptive research.Using the form of random distribution of questionnaires to investigate the basic situation of 9 community health service institutions in an administrative area of Xi’an.To know about the layout of emergency space and the facilities,equipment,medicines,the composition of medical and technical personnel and the training and learning situation.Investigate the basic situation,training and learning situation,self-assessment skills mastery and training needs,cognition of pre-hospital first aid and willingness to participate in the health technical staff of the institution.And investigate the basic situation of community residents,their knowledge of pre-hospital first aid,their recognitions of community health service institutions,and their satisfaction.3.Use Excel,SPSS18.0 and other related software for data processing and statistical analysis.Counting data were expressed in frequency and the measurement data is expressed by the mean plus or minus the standard deviation.Pearson chi-square test,Fisher exact probability method or continuous correction method were used for significance comparison.Spearman rank correlation analysis was used for correlation analysis.For all analyses,a pvalue of less than 0.05 was considered statistically significant.Research result:1.By Investigating the overall situation of 9 community health service institutions: the total area is large,the service radius is long and the service population is large.But all have no emergency department,emergency registration and emergency night shift.At the same time,all of them have not implemented a 24-hour duty system.Only 2 community health service organizations have independent emergency work areas,and 4 organizations are equipped with 1 ambulance by each.First aid equipment and medicines are not complete.There are a total of 110 physicians in 9 community health service institutions,and the average number of physicians per 10,000 population is about 2.1,of which the majority are over 40 years old.In 2019,the total number of trainees reached 175,mostly for community training and further study.All 9 community health service organizations have realized handover of patients with the paper version of higher-level hospitals.2.In the survey,85% of the medical technicians in community health service institutions were older than 40.74% have a bachelor degree or below.66% have no professional title or junior professional title.Among the medical technicians surveyed,94%of medical technicians said heard that the hospital first aid.And 86% of medical technicians think it is necessary to carry out community pre-hospital first aid.82% are willing to participate in community first aid.Statistics show that medical technicians with different ages,educational levels,professional titles,total working years,working hours in the unit and entry methods have significant differences in willingness to participate in community first aid,which is statistically significant.However,there is no significant difference in the willingness of medical technicians to participate in community first aid in different genders,marital status,positions and work styles.62% of the medical technicians who participated in the training believed that the training effect was good.Different training types have significant differences in training results,which are statistically significant.In addition,43%of medical technicians believe that they have mastered the monitoring of vital signs,and30% believe that they have mastered cardiopulmonary resuscitation techniques.Only 4%of medical technicians believe that they have mastered the operation of endotracheal intubation technology.More than half of the medical technicians said they needed to learn and train all the 7 basic first aid knowledge and skills in the questionnaire.3.Investigate residents within the jurisdiction of 9 community health service agencies.61.2% of those who have heard of pre-hospital first aid,82% of those who have heard of cardiopulmonary resuscitation.62% of residents are aware of defibrillators.According to statistical analysis,there are significant differences in residents’ perceptions of pre-hospital first aid in different ages,occupations,and marital status,which are statistically significant.And different gender,age,education level,work status,occupation and marital status have significant differences in the cognition of community residents in cardiopulmonary resuscitation,which is statistically significant.Only 1% of residents believe that they fully understand first aid knowledge.Over 71% of residents think they don’t understand or only understand a little.55.6% of residents indicated that they recognized the level of community first aid.Among them,90% of residents believe that community health service agencies can play a role in pre-hospital first aid.92.2% of residents think it is necessary to learn first aid knowledge.Residents who want to learn a variety of first aid knowledge accounted for38.3%.The higher demand is to judge common diseases and injuries,first aid for myocardial infarction,stroke,wound hemostasis,cardiopulmonary resuscitation and other related knowledge and skills.60.4% of the residents indicated that they prefer to learn and train relevant first-aid knowledge in community health service institutions.73.1% and 47% of residents said that first aid knowledge should be promoted in communities and schools.Conclusion:1.The investigation shows that the medical environment and condition of the community health service institutions in our city are well developed.But there are still some shortcomings,especially in front of pre-hospital emergency work.The country and the government need to guide the construction of a new model of pre-hospital first aid.Strengthen the medical capacity of community health service institutions.Gradually deprive the field first aid part to the grassroots.Establish a complete emergency medical service system suitable for our country.2.Insufficient first aid hardware equipment,medicines and funds in community health service agencies.Lack of knowledge and skills and high demand for learning and training are also important.Need to increase investment improvement of community health service agency hardware level.Further increase the number of community medical technicians and learning and training opportunities.Improve staff salary compensation mechanism.Attract more and better talents to stay at the grassroots level.3.Residents have poor recognition and satisfaction with community health institutions and pre-hospital first aid.Strong willingness to learn and train first aid knowledge.Attention should be given to the popularization and education of first aid knowledge in communities and schools,and the awareness of self-help and mutual rescue of the whole people should be improved. |
