Objective: The study aims to explore the mental health status of patients with polycystic ovary syndrome in Chongqing,and explore the factors that may affect their mental health from the aspects of disease characteristics,lifestyle,sleep status and endocrine characteristics,so as to provide scientific basis for improving the mental health of patients.Methods: The study on mental health of patients with polycystic ovary syndrome of childbearing age selected 433 patients who were first diagnosed with PCOS at gynecology department between July 2018 and January 2020 in two third-class hospital in Chongqing.At the same time,non-PCOS women of the same age who went to the gynecological outpatient department or the physical examination center for examination were selected as the control group,the mental health status was compared between the case and control group.The demographic characteristics(such as age,education level,occupation,cigarette and alcohol use,marital status),history of gynecological,history of fertility,and so on were measured by self-made questionnaires.Sleep-related problems were measured by the Pittsburgh Sleep Quality Index(PSQI),mental health status was assessed by the Hospital Anxiety and Depression Scale(HADS).Blood glucose,insulin,sex hormones,blood lipids and basic follicular information(follicle size,number of follicles,ovarian and uterine blood flow)were collected in the case group.The mental health status of 433 patients was described,the mental health status of patients in different age groups,different occupation groups,different places of residence,and different education levels were compared by chi-square test or Fisher’s exact probability test.Paired chi-square test,paired rank sum test,single factor and multivariate conditional logistic regression analysis were used to compare the mental health status of PCOS patients and the control group.In the study on influencing factors of mental health in patients with polycystic ovary syndrome,433 patients who were first diagnosed with PCOS at gynecology department between July 2018 and January 2020 in third-class hospital in Chongqing were included.The chi-square test,rank sum test,t test,univariate and multivariate logistic regression analysis were used to study the risk factors that may affect anxiety and depression in PCOS patients.Results: Part 1:(1)Analysis of the mental health status of 433 patients showed that anxiety-positive people accounted for 26.6%,and depression-positive people accounted for 23.6%.There was no difference in the rate of anxiety/depression-positive in PCOS patients of different ages,different education levels,and different BMI levels(all P>0.05).The rate of anxiety-positive in rural patients was higher than that in urban patients(P=0.043).(2)174 pairs of case-control study showed that there was no statistically significant difference in the risk of anxiety between the PCOS group and the control group(OR=1.488,95%CI: 0.861-2.571).The risk of depression in the PCOS group was higher than that of the control group(OR=2.403,95%CI: 1.315-4.392).(3)The sleep status of PCOS group was worse than that of the control group.The score of sleep disorder(OR=1.960,95%CI: 1.261-3.046),score of daytime dysfunction(OR=1.875,95%CI: 1.415-2.484)in PCOS group were all higher than those in the control group.Part 2:(1)A significant relationship was found between mental health and sleep status in patients of PCOS.The scores of sleep quality(OR=1.611,95%CI: 1.147-2.261),sleep disorder(OR=2.326,95%CI:1.468-3.685)and daytime dysfunction(OR=1.457,95%CI: 1.122-1.891)in the anxiety-positive group were higher than those in the anxiety-negative group.Similarly,the scores of sleep quality(OR=1.467,95%CI: 1.043-2.063),sleep disorder(OR=1.624,95%CI: 1.030-2.561)and daytime dysfunction(OR=1.406,95%CI: 1.077-1.836)in the depression-positive group were higher than those in depressed-negative group.(2)No relationship was found between the mental health stutas and disease characteristics(hairy,acne),as well as endocrine indicators(blood glucose,insulin,sex hormones,blood lipids and so on)in PCOS(all P>0.05).Conclusions: PCOS patients have poor mental health and sleep status,and sleep-related problems may be related to the occurrence and development of mental health problems in patients with polycystic ovary syndrome.We recommend that the comprehensive management of sleep problems should be part of the care of patients with PCOS. |
