| Objective: We conducted a survey on the causes of death of the Jinchang cohort from 2011 to 2020,to reveal the rank of death causes of employees,to explore the burden of disease deaths in the Jinchang cohort,to discover the core diseases that affect the health of the occupational population,to determine the key areas of disease prevention and control and high-risk groups,and to provide an important basis for health interventions for the occupational population.Methods: Using the cohort study method,a ten-year survey of all causes of death was conducted on all the people included in the cohort.The team designed the "2011-2020 Jinchang Cohort population Statistics Table" and the "2011-2020 Jinchang Cohort population All Causes of Death Registration Table" to investigate the work unit of the research population and obtain relevant data used to analyze the burden of disease for ten consecutive years.Obtained data included: information on-the-job employees,information on retired employees,information on deaths of on-the-job employees,and information on deaths of retired employees.The classification of causes of disease death is coded by ICD-10,and the calculation indicators included: crude death rate,standardized death rate,proportion of death causes,rank of death causes,life expectancy,potential years of life lost,work potential years of life lost,probability of premature mortality,etc.For major diseases in 10 years,Joinpoint regression was used to analyze the annual percentage change of mortality,standardized mortality,and probability of premature mortality,and test whether the change trend is statistically significant.The mortality rate was standardized using the data of the sixth national census in 2010.Results: 1.A total of 3754 deaths were reported in Jinchang from 2011 to 2020,including 3151 males,accounting for 83.94%,and 603 females,accounting for16.06%;The total population mortality rate was 766.57/100,000,the standardized mortality rate was 428.71/100,000,the male mortality rate was 1036.73/100,000,the standardized mortality rate was 507.85/100,000;The female mortality rate was324.58/100 000,the standardized mortality rate was 242.60/100 000,the male mortality rate was 3.19 times that of the female;The mortality rate of males,females,and the total population during the ten years showed an upward trend(P<0.05).2.From 2011 to 2020,the Jinchang cohort has the largest number of deaths from the second category of diseases(Chronic non-communicable diseases),with 3259 deaths,the mortality rate of 665.49/100,000,and the standardized mortality rate of365.06/100,000,accounting for 86.81% of the total deaths.The mortality rate of the second category of diseases for men and women and the total population during the ten years showed an upward trend(P<0.05).3.The top 6 causes of death in Jinchang cohort from 2011 to 2020 were circulatory system diseases,tumors,respiratory system diseases,endocrine,nutritional and metabolic diseases,injuries and digestive system diseases;The top 6male diseases were circulatory system diseases,tumors,respiratory system diseases,endocrine,nutritional and metabolic diseases,injuries and digestive system diseases.Among women,the top 6 diseases were tumors,circulatory system diseases,respiratory system diseases,endocrine,nutritional and metabolic diseases,injuries and digestive system diseases.4.The mortality of circulatory system diseases,heart diseases and coronary heart diseases in the Jinchang cohort population during the ten years showed an upward trend(P<0.05).5.From 2011 to 2020,the tumors mortality rate in Jinchang cohort was252.19/100,000,and the standardized mortality rate was 131.18/100,000,accounting for 32.90% of all causes of death.The tumors mortality and standardized mortality showed no increasing trend during the decade.The top five causes of death of tumors were lung cancer,gastric cancer,liver cancer,colorectal cancer and esophageal cancer.6.From 2011 to 2020,the mortality rate of respiratory diseases in Jinchang cohort was 74.74/100,000,and the standardized mortality rate was 42.24/100,000.The top three causes of death of respiratory diseases were COPD,silicosis and viral pneumonia.7.Diabetes was the first cause of death in the Jinchang cohort of endocrine,nutritional and metabolic diseases during the ten years,and the mortality rate was27.16/100,000,accounting for 91.72% of the causes of death of endocrine,nutritional and metabolic diseases.8.From 2011 to 2020,the top four causes of death from injuries were traffic accident,accidental fall,accident death at work and suicide,accounting for 35.29%,25.00%,22.79% and 5.88% of the total death from injuries,respectively.These four causes of death accounted for 88.97% of the total death from injuries.9.From 2011 to 2020,after the elimination of tumors in Jinchang cohort,the increase of life expectancy at all ages was the highest,followed by circulatory system diseases.10.From 2011 to 2020,the work potential years of life lost due to all deaths of female on-the-job employees were 752.50 person-years,and the average work potential years of life lost were 8.09 years;The work potential years of life lost due to tumors were the largest,followed by injuries,and the average work potential years of life lost due to injuries and digestive system diseases were the largest.The work potential years of life lost all deaths of male on-the-job employees were 4,317.50person-years,and the average work potential years of life lost were 10.06 years;The work potential years of life lost due to tumors were the largest,followed by circulatory system diseases;and the average work potential years of life lost due to endocrine,nutritional and metabolic diseases were the largest,followed by injuries.11.From 2011 to 2020,the potential years of life lost caused by all causes of death in the Jinchang cohort were 17,187.38 person-years,and the average years of life lost were 12.22 years;Tumors caused the largest number of potential years of life lost,followed by circulatory system diseases;Injuries caused the largest number of average potential years of life lost.12.From 2011 to 2020,the probability of premature mortality of four major chronic diseases in Jinchang cohort was 11.50%,14.81% in males and 6.46% in females.Malignant tumors(6.39%)had the highest probability of premature mortality,followed by cardio-cerebrovascular diseases(4.34%).The probability of premature mortality of four chronic diseases was higher in males than in females.In females,the probability of premature mortality from cardiovascular and cerebrovascular diseases during the ten years showed an upward trend(P<0.05).Conclusion: 1.Chronic non-communicable diseases,such as cardiovascular and cerebrovascular diseases,malignant tumors,diabetes and other diseases,are the most important problems affecting the health of the Jinchang cohort,and the mortality rate of chronic diseases is on the rise.2.Circulatory system diseases ranked first in the order of all causes of death,and the mortality showed an increasing trend during the decade.Heart disease,cerebrovascular disease is the main cause of death of circulatory system diseases,the mortality of heart disease is increasing,is the focus of future disease prevention and treatment.3.Tumor was the second most common cause of death,leading to the largest loss of survival time in the Jinchang cohort.Lung cancer was the first cause of death in this cohort,and the mortality of lung cancer showed no increasing trend,but the mortality rate was still high.4.Retired workers and male workers are the key groups for disease prevention in Jinchang cohort.5.The age of death group caused by injury is relatively young,and the traffic accident is the biggest threat to life in injury,the elderly are easy to fall to death,and attention should be paid to the threat of job death to the life safety of employees. |
