Clinical Observation Of Xiaoyao Tongxie Decoction Combined With Pivironium Bromide In The Treatment Of IBS-D(Liver Stagnation And Spleen Deficiency)

Objective:Based on the understanding of diarrhea-predominant irritable bowel syndrome in ancient and modern medicine,the pathogenesis of “ liver depression and spleen deficiency ” was taken as the breakthrough point,and the efficacy and safety of Xiaoyaotongxie Decoction combined with Pivaverium Bromide in the treatment of diarrhea-predominant irritable bowel syndrome(IBS-D)with liver depression and spleen deficiency syndrome were preliminarily discussed,which provided certain clinical basis for the treatment of IBS-D(liver depression and spleen deficiency syndrome)with integrated traditional Chinese and western medicine.Methods: 76 patients were divided into two groups according to the combination of Chinese and western medicine treatment and western medicine treatment,38 cases in each group,patients need to meet the conditions : from January 1,2020 to December 31,2020 in The First People’s Hospital of Yunnan Province gastroenterology and Yunnan Hospital of Traditional Chinese Medicine spleen and stomach diseases were diagnosed as diarrhea type irritable bowel syndrome(liver stagnation and spleen deficiency syndrome)outpatient and inpatient.The patients were grouped according to their wishes.The Combination group of Chinese and Western medicine was treated with Xiaoyaotongxie Decoction(2 times / day,1 dose / day)combined with Pinaverium bromide(po 50 mg tid),while the The western medicine group was treated with Pinaverium bromide(po 50 mg tid).Both groups were treated for 4 weeks.The improvement of the efficacy and safety indexes of Combination group of Chinese and Western medicine and The western medicine group were compared before treatment and after treatment,and the safety and clinical efficacy value of Xiaoyao Tongxie Decoction combined with pivironium bromide was discussed through statistical analysis.Results:There was no significant difference in the general situation(vital signs,age,gender,and course of disease)between the two groups before treatment(P > 0.05),indicating comparability.1.The total effective rate was 94.59 % in the Combination group of Chinese and Western medicine and 57.14 % in the The western medicine group(P < 0.01).2.The total score of TCM syndromes : the two groups were significantly improved after treatment(P < 0.001),and the Combination group of Chinese and Western medicine was superior to The western medicine group(P < 0.05).3.The frequency of defecation,character and abdominal pain : the two groups were significantly improved after treatment(P < 0.001),and the Combination group of Chinese and Western medicine was superior to The western medicine group(P < 0.05).4.Each single TCM symptom : After treatment,both groups were improved compared with those before treatment(P < 0.05).The efficacy of the Combination group of Chinese and Western medicine was superior to The western medicine group in improving the four symptoms of poor appetite,increased stool frequency,abdominal distension and abdominal pain,and loose stool(P < 0.05),but there was no difference in the improvement of belching and bowel sounds(P > 0.05).5.Depression and anxiety : Both groups were improved after treatment(P < 0.001),and the Combination group of Chinese and Western medicine was superior to The western medicine group(P < 0.05).6.The Combination group of Chinese and Western medicine and The western medicine group had no obvious adverse reactions.Conclusion: Xiaoyao Tongxie Decoction combined with pinaverium bromide is more effective than pinaverium bromide alone in the treatment of irritable bowel syndrome diarrhea(IBS-D)with liver depression and spleen deficiency syndrome.It can effectively relieve the symptoms of increased stool frequency,loose stool,abdominal distension and abdominal pain,anorexia,improve depression and anxiety of patients,and has good safety,which is suitable for clinical use and promotion.
