| Objective:To observe the effect of acupuncture and moxibustion combined with modern rehabilitation training on functional recovery of patients with rotator cuff tear. Methods:A total of 60 patients with medium and large rotator cuff tear were selected from the Department of Osteoarthritis,Affiliated Hospital of Hubei University of Chinese Medicine,Wuhan Hospital of Integrated Chinese and Western Medicine from December 2018 to December 2020 who underwent shoulder arthroscopic double-row suture bridge technique.Among them, there were 34 males,26 females,24 on the left shoulder,36 on the right shoulder,38 patients with a history of shoulder trauma, and 22 patients without obvious inducement.According to random number table method, they were divided into study group and control group, with 30 patients in each group.Control group received shoulder arthroscopic surgery after the modern rehabilitation training method for treatment;Patients in the study group were treated with acupuncture on the basis of modern rehabilitation training. In the study group, three acupoints,Shoulder Condyle,Shoulder Liao and Shoulder Zhen, were selected as the main acupoints in the first month after surgery, and acupuncture treatment was performed with Bugu and Houxi.1 month to 3 months after surgery,Shoulder Condyle,Shoulder Zhen and Shoulder Liao acupoints were selected for acupuncture treatment with Tianzong and Yanglingquan.Three months after the surgery-six months after the surgery, acupuncture treatment was performed by selecting Shoulder Condyle,Shoulder Zhen and Shoulder Liao with Tiekou and Zusanli. In the first month after surgery, acupuncture was performed twice a week, and the needle was kept for 20 minutes after each qi.During1-6months after surgery, acupuncture was performed once a week, and the needle was kept for 20 minutes after each qi.Two groups of patients in the preoperative and postoperative 1 month,3months after operation, postoperative 6 months using the UCLA shoulder joint rating of the university of California system,Europe shoulder joint association Constant-Murley shoulder joint scoring system, visual analogue scale(VAS)for shoulder function and pain assessment, and 12 months after surgery line shoulder magnetic resonance imaging, observe two groups of patients with rotator cuff healing after surgery.Result:The follow-up rate of patients in both groups reached100%.The general information(gender, age, inducement, tear site and tear range)of patients in both groups were statistically analyzed and compared, and there was no significant difference(P>0.05).There were no significant differences in preoperative pain VAS score,UCLA score and Constant-Murley score between 2groups(P>0.05).UCLA score,Constant-Murley score and pain VAS score in the study group were significantly better than those in the control group(P < 0.05)at the follow-up 1 month and 3 months after surgery.UCLA score,Constant-Murley score and pain VAS score in 2 groups were significantly better than preoperative score in the group at 1 month,3 months and 6 months after surgery(P<0.05).At6 months after operation, there were no significant differences in UCLA score,Constant-Murley score, pain VAS score between 2 groups(P>0.05).12 months after surgery,MRI examination of shoulder joint was completed, and there was no significant difference in rotator cuff healing between the two groups(P>0.05).Conclusion:Double row suture bridge technique under arthroscopic shoulder is a safe and effective method for the treatment of middle and large rotator cuff tear.Although postoperative follow-up 6months and 12 months after the results show that after arthroscopic shoulder surgery for acupuncture and moxibustion with modern rehabilitation therapy and simply give modern rehabilitation treatment of shoulder joint function and rotator cuff healing influence not big difference, but 1 month after surgery and postoperative follow-up of 3 months, found that patients with team shoulder function significantly better than the control group patients, shows that acupuncture and moxibustion in combination with modern rehabilitation training in patients with early postoperative can better ease pain, improve shoulder joint function in patients with activity. |
