ObjectiveLong-term follow-up of patients with rotator cuff tear and osteoporotic treated with arthroscopy to evaluate the efficacy of arthroscopic rotator cuff repair.MethodsChoose patients who got shoulder pain for more than one year,after conservative treatment is ineffective,MRI diagnosis for the full-thickness of tear and arthroscopic re-confirmed for the full-thickness of tearing.Exclusion criteria:Patients with large or partial-thickness rotator cuff tears;patients with cervical spondylosis and brachial plexus injury;traumatic shoulder dislocation or definite history of trauma;history of blockage therapy and frozen shoulder.From January 2015 to June 2018,a total of 45 patients with arthroscopic rotator cuff repair were completed by the research team,including 22 cases of(14 female,8 male)and 23 cases of non-(13female,10 male)who were diagnosed by double energy X-ray bone density before operation.The follow-up time was up to February 2019,1 cases(female)were lost in the osteoporosis group and 1 case(male)were lost in the normal bone mineral density group.We record the visual score of pain(PVAS),United States shoulder elbow surgeon score(ASES),Constant score and range of activities preoperative,3 months after surgery and 6 months after surgery.At the last follow-up,we use ultrasonography to detect the re-tear and compare the tearing rates of the two groups.ResultsThe pain visual analog scale(PVAS)in both groups was significantly lower than that before surgery,and the American Shoulder and Elbow surgeon scale(ASES),Constant score and activity range were significantly improved.There were no statistical differences in PVAS score,ASES score,Constant score and range of activity between the two groups 3 and 6 months after surgery.Six months after surgery,ultrasonography showed 3 cases of re-tearing in the osteoporosis group,and 1 patient got re-tear in normal BMD group.No rivet was pulled out in both group.ConclusionThe clinical effect of arthroscopic treatment of rotator cuff tear complicated with osteoporosis is accurate,but the risk of rotator cuff re-tear is higher than in patients with normal bone density In order to reduce the pain and improve the quality of life of patients,shoulder-arthroscopic rotator cuff repair is still an effective treatment for patients with rotator cuff injury combined with osteoporosis.
