Purpose We investigated and understood the quality of life of women with menopausal syndrome around 40-60 years old in a maternal and child health hospital in Nanjing.We explored the main influencing factors of the quality of life of women with perimenopausal syndrome and analyzed the correlation between peri-menopausal symptoms and quality of life.Based on this,group psychological therapy and Internet interaction feedback were used to psychologically intervene in perimenopausal women,evaluate the effect of intervention,analyze the factors affecting the intervention effect,and provide scientific basis for improving the quality of life of perimenopausal women.Method In this study,from June 2016 to June 2018,a questionnaire survey was conducted on460 perimenopausal patients with a perimenopausal outpatient case in a maternal and child health care center in Nanjing.The questionnaire mainly includes the improved kupperman rating scale,perimenopausal quality of life scale(MENQOL)and self-designed general situation questionnaire(general demographic sociological characteristics,health-related behavior)to analyze the status and influencing factors of life quality of patients with perimenopausal syndrome.On this basis,the subjects were randomly divided into the intervention group and the control group.The intervention group performed group psychotherapy once every week,and conducted Internet interaction feedback every 2 days for a three-month psychological intervention.The control group did not have special psychological intervention.The data was recorded by Epidata3.1 for double-machine entry,and statistical analysis was performed using SPSS21.0 statistical software.We used descriptive studies(mean ± standard deviation,percentage)to analyze the general condition and quality of life of patients with perimenopausal syndrome.We analyzed the correlation between perimenopausal symptoms and quality of life using the Pearson correlation coefficient.Evaluation of group psychological therapy and internet interaction feedback in patients with perimenopausal syndrome by t-test and chi-square test.The multiple stepwise regression analysis was used to analyze the influencing factors of quality of life and psychological intervention in women with perimenopausal syndrome.Logistic regression was used to analyze the influencing factors of behavioral changes.Result(1)The average age of perimenopausal syndrome patients in this study was48.87±3.57 years old,and the majors were undergraduate and above.There are 300 people(65.22%)who regularly participate in group activities,323 people(70.22%)who often eat soy products,275people(59.78%)who often eat dairy products,119 people(25.87%)who exercise basiclly every day.(2)Among the perimenopausal syndrome patients,the main perimenopausal symptoms are:insomnia(90.22%),fatigue and weakness(87.17%),hot flashes(86.74%),headache(77.6%),bone and joint muscle pain(84.57%).The perimenopausal symptoms were mainly moderate symptoms,accounting for 51.09% and the average score of the modified kupperman score was 27.49±4.48.Influential factors of perimenopausal symptoms showed: perimenopausal symptoms were related to age,low education level,early menstrual disorders,early participation in group activities,and infrequent eating of dairy products(P<0.05).(3)The comprehensive quality of life quality of patients with perimenopausal syndrome was36.28±17.32.Peri-menopausal symptoms were significantly positively correlated with quality of life,and insomnia and depressive symptoms were highly positively correlated with quality of life.The analysis of the influencing factors of the comprehensive evaluation of quality of life shows that the comprehensive score of quality of life is related to the improved kupperman score,whether to eat soy products,whether to eat dairy products,sports,and their annual income.The quality of life of perimenopausal syndrome patients with low scores of kupperman,frequent consumption of soy products and dairy products,high frequency of exercise,and high annual income was better(P<0.05).(4)The results of group psychological intervention in perimenopausal women show:After group psychological intervention.The vasomotor symptoms of the quality of life-related vasomotor symptoms in the intervention group were better than those in the control group(P<0.05).The difference between the baseline and the end of the relevant indicators in the intervention group was compared with the difference between the baseline and the end of the control group.After group psychological intervention,the intervention group showed significant reduction in perimenopausal symptoms,vasomotor symptoms,psychosocial status,physiological status,sexual life and comprehensive quality of life score while the control group score increased.The difference was statistically significant(P<0.05).The difference between perimenopausal symptoms,vasomotor symptoms dimension,psychosocial state dimension,physiological state dimension,sexual life dimension and comprehensive score in the final intervention group and the final control group was larger than the baseline difference,and the difference was statistically significant(P<0.05).(5)The multiple linear regression model was used to analyze the influencing factors of group psychological intervention.The results showed that the age of late menstrual disorders,the severe perimenopausal symptoms at baseline,and the improved kupperman score were significant,the difference was statistically significant.Its group psychological intervention is better.Changes in the scores of life quality dimensions: baseline scores of life quality have a greater impact on the changes in the final scores of each dimension.The higher the score,the more serious the baseline symptoms,the greater the change in quality of life scores(P<0.05).The scores of the high-education population in the vasomotor symptoms dimension and the psychosocial state dimension were significantly higher than those in the low-education population,and the difference was statistically significant(P<0.05).The scores of psychosocial state dimensions,physiological state dimensions and sexual life dimensions were significantly higher in those with early menstrual disorders than those with early menstrual disorders(P<0.05).The higher the annual income of the family,the greater the progress of the physiological state dimension score,and the better group psychological intervention.(6)After 3 months of group psychological intervention,patients with poor baseline health-related behaviors showed no change in behavior or even backward behavior.The results of analysis of factors affecting health-related behavioral changes using multiple linear regression models show:changes in health behavior were significantly associated with education and menstrual disorders(P<0.05).Group psychological intervention had no significant effect on the changes of healthy behaviors of patients with low education level and early menstrual disorders.Conclusion This study shows that women with perimenopausal syndrome have severe perimenopausal symptoms.The quality of life is poorly evaluated in terms of vasomotor symptoms and physiological state dimensions,and perimenopausal symptoms are severe,and soy products and dairy products are not often eaten.Low and low incomes can lead to a decline in quality of life.After 3 months of group psychotherapy combined with Internet interactive feedback intervention,the indicators in the intervention group were significantly improved,which fully demonstrated the effectiveness of the intervention program in improving the quality of life of women with perimenopausal syndrome.At the same time,the intervention effect of health-related behavior still has room for improvement,and the behavioral transition theory can be combined to improve the intervention mode for women with perimenopausal syndrome. |
